“Z”型支架对食管粘膜通透性的影响

目的 :研究实验犬食管“Z”型支架术后局部粘膜对聚乙二醇 40 0 (PEG40 0 )通透性的影响。 方法 :选择成年健康犬 12只 ,均分为 2组 ,分别置入“Z”型食管支架 ,支架术后 2 h选择第 1组实验犬进行食管持续恒压 PEG40 0 灌流 ,2 h后取出置架部位的食管组织及等量正常食管组织 ,通过气相色谱分析法检测组织中 PEG40 0 的含量 ,并进行比较。第 2组实验犬于术后 2 4h同样方法进行气相色谱分析。结果 :正常食管粘膜对 PEG40 0 无通透性 ,支架术后 2 h食管组织中 PEG40 0 的含量较正常明显升高 ,主要表现为一些较小分子的 PEG40 0 ;术后 2 4h PEG40 0 的通透性进一步增强 ,可检测到一些较大分子的 PEG40 0 ;支架中部食管组织 PEG40 0的总吸收值为 (96 .32± 9.6 1) mol/ g,较术后 2 h的 (34.2 4± 5 .81) m ol/ g显著升高 (P<0 .0 1)。 结论 :支架置入可导致食管局部粘膜对 PEG40 0 通透性显著升高 ,从而为外源性致病原的介入创造条件。

Objective:To study the changes of esophageal mucosa permeability after “Z” stenting.Methods: Twelve adult healthy dogs were divided into 2 groups, and the esophageal stent were placed respectively. Two hours after stenting PEG 400 was poured into the esophagus continuously under constant pressure in the first group 2 hours later. Equal weight of stenting and adjacent normal tissue were taken out, homogenated, centrifugated, the amount of PEG 400 in the supernatant was measured by gasliquid chromatography. Twenty four hours after stenting the same test was carried out in the second group. Results: Normal esophageal mucosa had no permeability for PEG 400 . The amount of PEG 400 in stenting esophagus was much higher than that of normal esophagus 2, 24 hours after stenting, the absorbing value of PEG 400 in middle esophageal tissue with stent was ( 96.32±9.61) mol/g, much higher than 2 h (34.24±5.81) mol/g .Conclusion: The permeability of esophageal mucosa for PEG 400 increase significantly after stenting. It also indicates that stenting may impair the esophageal mucosa.
