电解可脱性铂弹簧圈栓塞治疗颅内动脉瘤26例

ENDOVASCULAR TREATMENT OF INTRACRANIAL ANEURYSMS WITH GDC

报告使用电解可脱性铂弹簧圈 (Guglielmi detachable coil,GDC)栓塞治疗颅内动脉瘤的情况。方法 :气管内插管全麻和肝素抗凝下 ,使用 Seldinger's技术 ,经 Tracker微导管放置 GDC栓塞颅内动脉瘤。结果 :成功栓塞 2 6例 31个颅内动脉瘤 ,其中 2 7个为 10 0 %栓塞 ,3个为 95 % ,1个为 90 %。3个 95 %栓塞均为宽颈动脉瘤。1个 90 %栓塞者 ,为 Hunt分级 级 ,存在严重脑血管痉挛 ,经解痉治疗效果不好 ,而放弃继续栓塞。无并发症及死亡。结论 :GDC是栓塞颅内动脉瘤较理想的方法 ,具有安全可靠、效果确切 ,适合颅内动脉瘤的治疗 ,尤其适合栓塞危险性较大的动脉瘤。

Objective:To assess the usefulness and practical experience in intracranial aneurysm embolization with guglielmi detachable coil (GDC) Method:Under general anesthesia and systematic anticoagulation, 31 cases of intracranial aneurysms were embolized with GDC which was introduced by tracker micro catheter Results:100% occlusion was achieved in 27 cases,95% in 3 and 90% in 1,without any complication Among these cases,3 were wide neck intracranial aneurysms and 1 failed in the attempts of endovascular treatment with serious vasospasm Conclusion:Embolizasion of intracranial aneurysms with GDC is safe,effective and reliable,and it may offer the cure opportunity to those giant aneurysms which are very dangerous to treat
