2型糖尿病患者24小时尿8-异前列腺素F2a及血清胱抑素C与肾脏损害的关系研究 被引量:5

Relationship between 24-hour Urinary 8-iso-PGF2a,Cystatin C and Renal Damage in Patients with Type 2 Diabetes Mellitus

摘要 目的探讨2型糖尿病患者体内氧化应激、胱抑素C(Cys C)水平变化特点及与肾脏损害的相关性。方法选取120例2型糖尿病患者,按照24 h尿微量清蛋白定量将患者分为A组(尿微量清蛋白<30 mg/24 h,60例)和B组(尿微量清蛋白30~299 mg/24 h,60例);另选取40例健康体检者作为对照组(C组)。比较3组受检者的体质指数(BMI)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、血清Cys C(Cys C)、24 h尿8-异前列腺素F2a/肌酐(8-iso-PGF2a/Cr)。结果 A组、B组患者血清Cys C水平及24 h尿8-iso-PGF2a/Cr均显著高于C组(P<0.01),且B组又显著高于A组(P<0.01)。多元逐步回归分析显示,24 h尿8-iso-PGF2a/Cr与血清Cys C水平呈正相关(r=0.674,P<0.01)。结论 Cys C、24 h尿8-iso-PGF2a/Cr均在糖尿病患者中明显升高,其升高程度与肾脏损害程度一致。 Objective To study the relationship between 24-hour urinary 8-iso-PGF2a,serum Cystatin C(Cys C) and renal damage in patients with type 2 diabetes mellitus.Methods 120 patients with type 2 diabetes mellitus were divided into group A(microalbuminuria〈30 mg/24 h,60 cases) and group B(30 mg/24 h≤microalbuminuria≤299 mg/24 h,60 cases) according to the level of 24-hour microalbuminuria.40 healthy physical examiners were involved as control group(group C).BMI,FPG,HbA1c,Cys C and the value of 8-iso-PGF2a/Cr were compared in the three groups.Results The levels of Cys C and the value of 24-hour urine 8-iso-PGF2a/Cr in group A and group B were significantly higher than those of group C(P〈0.01),and those of group B were also significantly higher than those of group A(P〈0.01).Multiple stepwise regressions showed that 24-hour urine 8-iso-PGF2a/Cr was positively correlated with the level of serum Cys(r=0.674,P〈0.01).Conclusion The level of serum Cys C and 24-hour urine 8-iso-PGF2a/Cr both increase in patients with type 2 diabetes mellitus and the increasing level is correlated with the severity of nephropathy.
