急性肺栓塞患者治疗前后肺血管阻力变化及其临床意义

Changes and clinical significance of pulmonary vascular resistance in patients with acute pulmonary embolism before and after treatment

目的探讨肺血管阻力(PVR)变化在评价急性肺栓塞(APE)患者治疗效果中的临床应用价值。方法回顾性分析30例APE患者治疗前后肺血管阻力变化并探讨其临床意义。结果 PVR在肺栓塞治疗前明显升高,治疗后2周、一月、三月、六月的PVR值与治疗前比较,降低明显,差异有统计学意义;其肺泡-动脉血氧分压[P(A-a)DO2]、PaO2值在治疗后2周、一月、三月、六月与治疗前比较,差异有统计学意义。结论超声心动图测定肺血管阻力结合动脉血气分析是APE治疗效果的良好识别指标。

Objiective To explore the Clinical value of the variation of the pulmonary vascular resistance（PVR） in patients with acute pulmonary embolism（APE）.Methods The variation of PVR in patients with APE before and after treatment were reviewed.Results The PVR in patients with APE before treatment significantly increased.The PVR in patients with APE had more significantly decrease 2 weaks,1 month,3 months,6 months after treatment and there were statistical differences between before and after treatment.There were statistical differences in alveolar-arterial partial pressure of oxygen,PaO2 between before and 2 weaks,1 month,3 months,6 months after treatment.Conclusions The PVR combined with arterial blood gas analysis be considered as sensitive index for the clincal effect of APE.
