肺癌根治术后呼吸衰竭51例治疗分析

The treatment for post-operative respiratory failure in patients with lung cancer

目的提高对肺癌根治术后呼吸衰竭患者的治疗水平。方法对肺癌根治术后呼吸衰竭51例患者,于ICU监测治疗;动态监测生命体征、肺功能、出血情况、血气分析;治疗合并疾病;纤维支气管镜吸痰;应用呼吸机进行无创或有创机械通气,治疗呼吸衰竭。结果 51例患者经过ICU加强诊疗,41例顺利恢复;6例死于多脏器功能衰竭,1例死于呼吸窘迫综合征,3例死于严重感染,死亡率19.61%。结论肺癌根治术后呼吸衰竭患者的诊疗中,动态监测生命体征、防治手术后并发症,积极应用气管镜吸痰及机械通气治疗,能有效治疗疾病,缓解病情。

Objective To improve the treatment of respiratory failure in patients with lung cancer after radical correction.Methods Clinical data of 51 patients with respiratory failure after resection of lung cancer were analyzed.All of them accepted the intensive care treatment after operation including continuous monitoring patient’s life signs,the bleeding amount,the function of the lung,the blood gas analysis,treating the complicated diseases,getting rid of sputum by bronchofibroscope,using ventilator.Results Through the treatment in ICU,35 patients smoothly recovered.4 patients died of MODS,1 patient died of ARDS,3 patients died of severe infection,and mortality rate was 19.61%.Conclusions During the treatment for patients who with respiratory failure after resection of lung cancer,continuous monitoring patient’s life signs,treating the complicated diseases,and using bronchofibroscope and ventilator can effectively improve the disease.
