Research Progress in Hospital-community Integrated Management on Diabetes in China

本文综述了糖尿病医院社区一体化管理的定义、管理模式、实施内容、发展中存在的问题并提出对策,以期为促进糖尿病医院社区一体化管理模式的完善和推广提供依据,以适应我国社会发展的需要。

In this paper we review the definition of hospital-community integrated management on diabetes mellitus,mode of the management,operating contents,existing problems during the development and their countermeasures,so as to provide basis for the improvement and popularization of the mode,with a purpose to adapt to the need of social development in China.
