厄贝沙坦联合比索洛尔对高血压合并阵发性心房纤颤患者窦性心律的维持及左心房重构的影响

Effect of combination of Irbesartan joint Bisoprolol on maintenance of sinus rhythm and left atrial structural remodeling in patients with hypertension together with paroxysmal atrial fibrillation

目的评价厄贝沙坦联合比索洛尔对高血压合并阵发性心房纤颤窦性心律的维持及左心房重构的影响。方法将86例高血压合并阵发性心房纤颤的患者随机分为治疗组和对照组,均给予比索洛尔治疗,治疗组加用厄贝沙坦,疗程均为1年。比较两组治疗后第3、6、18个月的窦性心律维持的比率和第6、18个月左心房内径的变化。结果治疗后第3、6个月,治疗组窦性心律的维持率分别为78.05%、73.17%,对照组窦性心律的维持率分别为78.57%、71.43%,两组间比较差异无统计学意义(P>0.05),治疗18个月后,治疗组的窦性心律维持率为70.73%,对照组为52.38%,两组间比较差异有统计学意义(P<0.05)。治疗后第6个月,治疗组左心房内径为(45.8±4.9)mm;对照组为(46.2±4.8)mm,两组间比较差异无统计学意义(P>0.05),治疗18个月后,左心房内径分别为(39.2±4.6)mm与(45.1±4.4)mm,两组间比较差异有统计学意义(P<0.05)。结论厄贝沙坦联合比索洛尔对高血压合并阵发性心房纤颤窦性心律的维持作用显著优于单用比索洛尔,并有逆转左心房重构的作用。

Objective To investigate the effect of combination of Irbesartan joint Bisoprolol on maintenance of sinus rhythm and left atrial structural remodeling in patients with hypertension together with paroxysmal atrial fibrillation. Methods Eighty-six patients with hypertension together with paroxysmal atrial fibrillation were randomly divided into the treatment group and control group, they all given Bisoprolol treatment, the treatment group added with Irbesartan, the period of treatment were all for 1 year. After treatment of the 3, 6, 18 months of sinus rhythm maintain ratios and the 6, 18 months left atrium of the inner diameter changes were compared between the two groups. Results After treatment of the 3, 6 months, the sinus rhythm maintain rate was 78.05%, 73.17% in the treatment group, and the sinus rhythm maintain rate was 78.57%, 71.43% in the control group, the difference was no statistically significant between the two groups （P 〉 0.05）, after treatment of the 18 months, the sinus rhythm maintenance ratio was 70.73% in the treatment group, the control group was 52.38%, the difference was statistically significant between the two groups （P 〈 0.05）. After treatment of the 6 months, the left atrium for inner diameter was （45.8±4.9） mm in treatment group, the control group for （46.2±4.8） mm, the difference was no statistically significant between the two groups （P 〉 0.05）, after treatment of the 18 months, the left atrium inner respectively was （39.2±4.6） mm and （45.h±4.4） mm, the difference was statistically significant between the two groups （P 〈 0.05）. Conclusion The combination of Irbesartan and Bisoprolol is more effective than Bisoprolol alone for sinus rhythm maintenance and may reverse left atrial structural remodeling
