早期绝经后妇女雄激素水平对体内脂肪分布的影响

Influence of androgen levels on the body fat distribution in early postmenopausal women

目的研究脂肪组织和雄激素水平在早期绝经后妇女体内的分布情况和二者的相关性。方法选择2009年7月～2010年6月到本院检查的70例绝经后妇女,根据体质指数划分为对照组35例,观察组35例,并检测入选人员身体各部分脂肪组成的情况,计算总脂肪含量、男性脂肪比例、女性脂肪比例及躯干脂肪量与下肢脂肪量的比值(T/L);分别抽取两组妇女的静脉血,通过离心机分离后,取上层清液。测定性激素结合蛋白(SHBG)、雌二醇、睾酮水平,使用ELISA法测定血清雌酮水平,计算出游离睾酮指数及雌二醇与雌酮的比值(E2/E1)。结果观察组体内脂肪量、总脂肪百分含量、男性脂肪比例、体质量、BMI均显著高于对照组,差异有统计学意义(P<0.01);观察组E2/E1及SHBG均低于对照组,FAI高于对照组,差异亦有统计学意义(P<0.01);观察组FAI与男性脂肪比例、T/L数值呈正相关性,同女性脂肪比例呈负相关性。结论绝经后女性体内雄激素的变化水平可能会影响妇女体内脂肪的分布,导致代谢疾病的发生。

Objective To study the distribution and correlation between adipose tissue and androgen levels in early postmenopausal women. Methods Seventy cases of postmenopausal women checked in our hospital from July 2009 to June 2010 were divided into the control group of 35 cases and observation group of 35 cases according to body mass index, and the body fat composition of parts were checked, the total fat content, the fat proportion of men, the fat proportion of women and the ratio of trunk fat mass and lower extremity fat mass （T/L） were calculated; venous blood of women in two groups were drawn, through the centrifuge separation, the upper clear liquid was taken. Sex hormone-binding （SHBG）, estradiol, testosterone level were determined, the level of serum female ketone was determined by ELISA, free testosterone index and estradiol and the ratio of female ketone （F-0/E1） were worked out. Results The body fat amount, the total fat percentage content, male fat percentage, body quality, BMI in the observation group were significantly higher than those in the control group, the difference was statistically significant （P 〈 0.01）; E2E1 and SHBG in the observation group were lower than that of the control group, FAI was higher than that in the control group, there were statistically significant differences （P 〈 0.01）; FAI in the observation group had positively relatation with the male fat percentage and T/L, and had negative relatation with female fat percentage. Conclusion The change levels of androgen may has the influence on the body fat distribution in early postmenopausal women, which lead to the metabolic disease.
