剖宫产围手术期3种抗菌药物预防感染的对比研究

Investigation of three antibiotics for prevention of infections during perioperative period of cesarean section

目的比较剖宫产围手术期3种抗菌药物预防感染的效果。方法选择医院择期剖宫产的产妇1290例,根据其围手术期用药随机分为3组,A组为青霉素组430例,B组为头孢替唑组430例,C组为头孢呋辛组430例,比较使用3种抗菌药物的临床效果、不良反应等,将结果进行统计分析。结果 B、C组术后体温恢复时间较A组短,B、C组产褥发病率、腹部切口感染率,分别为0.93%、0.23%,均较A组(1.63%、1.63%)低,差异有统计学意义(P<0.01);各组不良反应小,差异无统计学意义。结论头孢类抗菌药物在剖宫产术围手术期预防性应用抗菌药物控制感染的疗效肯定,基于抗菌药物应用的原则,临床应以头孢一代用药为首选。

OBJECTIVE To evaluate the effects of the three antibiotics on the prevention of infections during the perioperative period of cesarean section. METHODS A total of 1290 puerpera undergoing the selective cesarean section were selected as the study subjects, those patients were randomly divided into three groups, with 430 cases in each, according to the application of antibiotics during the perioperatvie period, the group A was treated with penicillin, the group B was treated with ceftezole sodium, and the group C was treated with cefuroxime sodium, the clinical efficacy and the adverse reactions of the three groups were compared, the statistical analysis of the result was performed. RESULTS As compared with group A, the group B and C needed less time to restore the body temperature, the puerperal morbidity and the incidence of abdominal incision infection of the group B and group C were 0.93% and 0.23%, respectively, both of which were lower than the group A （1.63% ,1.63%）, the differences were statistically significant （P〈0.01） ; there were few adverse reactions, the difference was not statistically significant. CONCLUSION Cephalosporins are effective in prevention of infections during the perioperative period of cesarean section, the first generation of cephalosporins should be selected as the first line drugs based on the principle of use of antibiotics.
