城市社区居民选择社区卫生服务影响因素研究

Study on influencing factor of choosing community health services for city community residents

目的分析影响上海市城市社区居民选择社区卫生机构服务的主要因素。方法采取整群随机抽样,在上海市某社区居民中抽取1 089名18岁以上居民进行问卷调查,应用二分类无序变量的多因素Logistic回归方法对数据进行处理,建立模型。结果在影响社区居民选择社区卫生机构就诊的因素中,性别、职业、医疗保险、每月经济收入、步行至社区卫生机构时间无统计学意义(P均>0.05);文化程度、年龄、2周内是否接受过社区卫生服务、是否患有慢性疾病有统计学意义(P<0.05或0.01)。结论年龄、文化程度、2周内是否接受过社区卫生服务及是否患有慢性疾病的交互作用是影响城市社区居民是否首先选择社区卫生机构的主要因素,这就要求社区卫生服务机构加强"六位一体"内涵建设,并根据社区人群的特点提供多层次、多样化的卫生服务。

Objective It is to analysis the main influencing factor of Shanghai city community residents to choose community health institutions. Methods Take a cluster random sampling, the residents in certain community of Shanghai city from 1089 men aged 18 years or older residents conducted a questionnaire survey, the data was processed by using two classification random variable multivariate logistic regression methods for, and established model. Results Affected community residents in community health institutions for treatment selection factors, gender, occupation, medical insurance, monthly income, time of walk to the community health institutions hadn＇ t statistical significance （ P 〉 0.05 ） ; but cultural level, age, received commu- nity health service in 2 weeks, and chronic disease had statistically significant （P 〈 0.05 or 0.01 ）. Conclusion The interac- tion of age, culture level, whether or not accepted community health services and whether patients with chronic disease was the main influenced factors to choose community health institutions in Shanghai city community residents, so community health service agencies must strengthen the ＂six in one＂ connotation construction, and provide multilevel, diversified health serv- ices according to the characteristics of community groups.
