超声引导下穿刺治疗小儿阑尾炎术后腹腔脓肿

Ultrasound- guided puncture aspiration for the treatment of post- appendectomy intra -abdominal abscess in children

目的探讨超声引导下抽吸或置管引流治疗小儿阑尾炎术后腹腔脓肿的价值。方法对35例小儿阑尾炎术后腹腔脓肿患儿行经超声引导下穿刺抽脓或置管引流。治疗后行超声随访。结果35例患儿共进行41次抽吸和2次置管引流，在超声引导下均一次穿刺成功抽脓，6例1周后进行第2次穿刺抽脓，均无严重并发症。35例患儿中31例均于抽吸引流后48h内症状缓解；4例穿刺抽脓后无明显改善，其中2例行置管引流后病情好转，2例中转开腹。结论超声引导下穿刺抽吸或置管引流治疗小儿阑尾炎术后腹腔脓肿是一种安全有效的治疗方法。

Objective To explore the value of ultrasound - guided pucture aspiration or catheter drainage for the treatment of intra - abdominal abscess following appendectomy in children. Methods 35 children with intra - abdominal abscess following appendectomy underwent ultrasound - guided puncture to aspirate pus or catheter drainage. Then follow - up was done after treatment. Results All of the 35 patients with 41 times of ultrasound - guided aspiration and 2 times of catheter drainage could be performed successfully for the first time. The second puncture was conducted in 6 ca- ses after 1 week. 31 patients who had been drained successfully showed symptomatic relief within 48 h. Of the 4 cases who showed no obvious improvement after aspiration, catheter drainages were performed in 2 patients and clinical symp- toms were improved, 2 cases received operation finally. Conclusion Ultrasound - guided pucture aspiration and catheter drainage is an effective and safe way in the treatment of post - appendectomy intra - abdominal abscess.
