摘要
本文报道了68只狗异体全胰、部分十二指肠、肾联合移植术。术中改进胰、肾血管常规的吻合方式,将移植物肾动、静脉与脾动、静脉末端吻合,使胰肾联成一整体,以肾脏替代脾脏,恢复胰、脾血管正常的血液循环途径。实验结果衰明:本术式简单、安全,可以改善移植物的血流状况,增加门静脉吻合口的血流量,血栓发生率较低,为临床胰肾联合移植手术方式的进一步改进提供了一个较为成功的动物模型。
For studying the method to prevent thrombosis in pancreatic graft, simultaneous transplantation of pancreas and kidney in 68 dogs were performed. We improved the routine transplant method. The artery and vein of the kidney in the graft were anastomosed end-to-end to the distal ends of artery and vein of the spleen in the graft, respectively. The kidney and pancreas were connected together and transplanted en bloc to the right iliac fossa in the recipient. Our experimental results have shown that this method, with the kidney instead of the spleen, can restore the normal way of blood circulation of the splenic vessels and increase the blood flow of the graft. The rate of graft thrombosis is low. It is of some clinical significance to further develop pancreaticorenal composite transplan-ation.
