经皮椎体成形术治疗老年胸腰椎压缩性骨折36例疗效分析

目的探讨经皮椎体成形术治疗老年骨质疏松性胸腰椎体新鲜压缩性骨折的临床疗效。方法对36个椎体行经皮椎体成形术(PVP),术前及术后3d进行疼痛视觉评分(VAS)和活动能力评分;随访期间观察患者疼痛缓解及生活能力状况,并分析骨水泥渗漏等并发症。结果手术全部成功,术后早期疼痛明显缓解,VAS评分:术后3d[(2.2±0.3)分]较术前[(6.2±0.3)分]明显下降(P<0.05);活动能力评分:术后3d[(1.2±0.5)分]较术前[(3.0±0.6)分]明显改善(P<0.05)。平均随访12个月,1例患者出院后半年因并发内科疾病死亡,其余患者治疗效果满意。结论掌握好手术适应证,术中精心操作,经皮椎体成形术是老年骨质疏松性胸腰椎新鲜压缩性骨折治疗的主要选择。

Objective To study the effect and complications of percutaneous vertebroplasty in treating osteoporotic spinal compression fractures of the ages.Methods A group of 36 patients underwent percutaneous vertebroplasty（PVP）.Visual analogue score（VAS） and mobility were evaluated 3 days before and after the operation.Relief of backache,recovery of living ability and complications were observed in the following up of 12 months.Results All patients were successfully operated and their backache relieved immediately after operation.Score of pain was distinctly decreased from（2.2±0.3） preoperation to（6.2±0.3） 3 days after operation with a significant difference（P〈0.05）.Score of patients’ mobility was distinctly increased from（1.2±0.5） preoperation to（6.2± 0.3）.3 days after operation with a significant difference（P〈0.05）.During average 12 months follow up,one patient died because of medicine disease after 6 months.All patients got satisfactory clinical effect.Conclusion Under strict selection and elaborate operation,PVP is an ideal method in treating osteoporotic spinal compression fractures of the ages.
