摘要
目的:探讨计算机辅助立体定向技术和颅内多靶点组合毁损对难治性精神分裂症的临床疗效及安全性。方法:选择80例难治性精神分裂症病患者作为研究对象,每例患者均在CT和MRI图像融合技术指导及计算机辅助下,采用杏仁核、伏隔核、内囊前肢、扣带回等多靶点组合射频热凝治疗。按全国精神外科协作组1990年制定的疗效评定方法进行疗效评定。结果:80例患者中,术后2周显著进步36例,进步41例,无效3例,加重0例,总有效率96.25%;术后6个月显著进步43例,进步33例,无效4例,加重0例,总有效率95%;术后无严重并发症和后遗症发生。结论:计算机辅助立体定向颅内多靶点毁损治疗难治性精神病安全有效。
Objective:To explore the curative effect and safety of the computer assisted stereotactic intracranial multi- targets stereotactic lesions on intractable schizophrenia. Methods: Under the guidance of computed X-ray tomography and magnetic resonance imaging image mergence and the computer assistance, the radiofrequency thermocoagnlations of intracerebral bilateral multi-targets including the cingulated gyri, nucleus accumbensa, mygdaloid nuclei and anterior limbs of the internal capsules and so on were performed in 80 patients with intractable schizophrenia, thermocoagula- tion. According to 1990 standards from National Psychosurgery Cooperation, the therapeutic outcome was assessed 2 weeks and 6 months after operatioru Methods.. After 2 weeks of operation to 80 cases: efficiency ratio 96.25 ~;after 6 months of opemtion to 80 cases: efficiency ratio 95 ~, no complication and sequela taken occurrence& Conclusion: The therapy, computer assisted stereotaetie intracranial multi-targets stereotactic lesions in the treatment of intractable schizophrenia, is safe and efficient.
