数字平板探测器DSA的各种常用模式在颅内动脉瘤术前评估的临床应用

摘要 目的应用三维数字减影血管造影(Digital subtraction angiography,DSA),对颅内动脉瘤进行诊断,详细了解颅内动脉瘤的解剖特征及血流动力学的改变,通过对数字平板探测器DSA的各种常用成像模式的对比分析,旨在合理有效的使用数字平板探测器DSA的各种成像模式对颅内动脉瘤进行精确诊断和评估。方法研究包含32例患者(38个动脉瘤),均为已经破裂过的动脉瘤,均进行过数字平板探测器DSA的检查,由两名具有高级职称的神经放射学家独立对3D-DA的各种常用模式图像和旋转DSA的图像进行评估。四分法系统被用来对动脉瘤进行诊断和术前评估,对比分析各种成像模式对动脉瘤的诊断和细节显示。结果对于被证实的38个动脉瘤的诊断,常规2D-DSA图像明显不如旋转DSA图像和3D-DA图像。对于动脉瘤的细节显示,旋转DSA图像明显不如3D-DA图像的各种常用模式,而3D-DA图像的各种常用模式之间基本没有显著性差异。结论对于颅内动脉瘤的术前评估,3D-DA图像比旋转DSA图像和2D-DSA图像能提供更多有用的信息。 Objective To use 3D digital tablet detector DSA,carries on the diagnosis of intracranial aneurysms, detailed understanding the anatomical characters of intracranial aneurysms, through of digital tablet detector DSA, comparison and analysis of the common imaging approach aimed at reasonable and effective use of digital tablet detector DSA of imaging mode intracranial aneurysm on preoperative diagnosis and evaluation. Methods Prospectie study contains 32 patients (38 aneurysm), is all the aneurysm is broken, underwent digital fiat detector DSA check, by two neuroradiologists independence evaluated for 3D-DA various kinds of image and rotation DSA mode of image. Four points law system is used to diagnosis and preoperative assessment aneurysm, c0ontrast analysis of various imaging mode on the diagnosis and detail shows for aneurysm. Results To be confirmed the detectiong of the 38 aneurysm, conventional 2D-DSA image obviously lower than rotating DSA image and 3D-DA image. For the delineation of the aneurysm, rotate DSA image obvious lower than 3D-DA image all kinds of common mode, and the image of various kinds 3D-DA between basically no significant difference. Conclusion For the preoperative assessment intracranial aneurysms, 3D-DA image than rotation DSA image and 2D-DSA image can provide more useful information.
