摘要
目的探讨T2DM患者血浆网膜素-1(Omentin-1)与颈动脉粥样硬化(AS)的关系,探讨其在T2DM颈AS及大血管病变中的作用。方法将62例T2DM患者根据颈动脉-内膜中层厚度(IMT)分为AS组和非AS组,同期选择我院健康体检者28名作为NC组,采用ELISA测定血浆Omen-tin-1水平,并分析血浆Omentin-1与BMI、血糖、血脂、高敏C反应蛋白(hsC-RP)、胰岛素抵抗指数(HO-MA-IR)等相关代谢指标的关系。结果 AS组与非AS组血浆Omentin-1水平均低于NC组[(14.90±6.27)vs(18.83±7.91)vs(24.80±9.15)ng/ml,P均<0.01];AS组血浆Omentin-1水平显著低于非AS组(P<0.01)。T2DM患者血浆Omentin-1与WHR、BMI、肝细胞生长因子(HGF)、hsC-RP、IMT、FPG、HOMA-IR、HbA1c呈负相关,与HDL-C呈正相关。HOMA-IR、BMI和IMT为影响T2DM患者血浆Omentin-1的独立相关因素。HOMA-IR、病程、hsC-RP和Omentin-1是影响T2DM患者IMT的独立相关因素。结论血浆Omentin-1与T2DM患者早期AS密切相关,可能在T2DM颈AS及大血管病变的发生发展中起着重要作用。
Objective To investigate the association between plasma omentin-1 and carotid atherosclerosis in type 2 diabetic patients and to explore roles of omentin-1 in carotid atherosclerosis and other macroangiopathy of type 2 diabetes mellitus.Methods Sixty-two patients with type 2 diabetes mellitus were divided into atherosclerosis(AS) group and non-AS group according to the intima-media thickness(IMT) of the carotid artery,and meanwhile 28 normal subjects from health examination were chosen as the control group.The fasting plasma omentin-1 level was measured by enzyme-linked immunosorbent assay(ELISA).The association of plasma omentin-1 level with body mass index(BMI),blood glucose and lipids,high-sensitivity C-reactive protein(hsC-RP),HOMA-insulin resistance index(HOMA-IR),and other metabolic parameters was also analyzed.Results The fasting plasma omentin-1 level in the AS and non-AS groups was lower than in the NC group [(14.90±6.27) vs(18.83±7.91) vs(24.80±9.15) ng/ml,P<0.01],and it was lower in the AS group than in the non-AS group(P<0.01).The fasting plasma omentin-1 level was correlated negatively with waist hip ratio(WHR),BMI,hepatocyte growth factor(HGF),hsC-RP,IMT,fasting blood glucose(FPG),HOMA-IR,and hemoglobin A1c(HbA1c),and positively with high density lipoprotein cholesterol(HDL-C).HOMA-IR,BMI,and IMT were the independent related factors in influencing the plasma omentin-1 level of T2DM patients.HOMA-IR,course of disease,hsC-RP,and omentin-1 were the independent related factors in influencing the IMT of the carotid artery of T2DM patients.Conclusion The plasma omentin-1 level is closely correlated with the premature atherosclerosis in T2DM patients,and it may play an important role in the occurrence and development of carotid atherosclerosis and other macroangiopathy in T2DM.
