乳腺癌术后患者行SPECT全身骨显像的技术影响因素 被引量:7

摘要 目的探讨乳腺癌术后患者在行单光子放射计算机断层成像(SPECT)全身骨显像时的各种技术因素对图像质量的影响。方法对635例乳腺癌术后患者静脉注射”99mTc-亚甲基二膦酸盐(MDP)后2-4h行SPECT全身骨显像,对静脉药物注射、检查体位的摆放、扫描条件的设定、引流管引流袋的放置、图像后处理、伪影的分辨等方面进行讨论和改进。结果由2名有经验的核医学医师判评,在药物注射、体位摆放、扫捕方式、伪影等方面进行评价,576例患者的图像非常满意,59例的图像存在缺陷。结论SPECT全身骨显像是乳腺癌术后患者诊断骨转移的常用方法,核医学技术员在日常操作中应尽量避免各种技术因素缺陷对图像质量产生的影响,努力确保显像质量。 Objective To explore the influence of various kinds of technological factors on image quality of whole body bone imaging in postoperative breast cancer patients. Methods The whole body bone imaging with SPECT was performed in 635 postoperative breast cancer patients 2-4 h after intravenous injection of 99"Tc-MDP. Technological factors influencing image quality, such as intravenous injection, selection of imaging position, setting of acquisition parameter, placing of drainage tube or bag, image post-processing, differentiation of artifacts, etc, were discussed and improved in this paper. Results The images were evaluated by two experienced nuclear medicine physicians, based on intravenous injection, selection of imaging position, setting of acquisition parameter, differentiation of artifacts, etc. 576 images were satisfactory, while 59 images were deficient. Conclusion The whole body bone imaging with SPECT is a useful method that is mainly used in postoperative breast cancer patients to diagnose bone metastasis. The nuclear medicine technicians must do their best to avoid the defects of various kinds of technological factors and ensure the image quality.
