近3年某院急诊药房不合格处方的调查分析

Investigation and Analysis of Unqualified Prescription in Emergency Pharmacy in Recent 3 years in A Hospital

目的调查分析急诊药房不合格处方,旨在为提高处方质量、促进临床合理用药提供参考。方法对我院2009年1月至2011年12月急诊药房处方进行调查,以《处方管理办法(试行)》为依据,分析不合格处方书写不规范和不合理用药情况。结果我院急诊药房2009年1月至2011年12月处方共22387张,其中不合格处方958张,不合格率为4.28%。不合格原因包括书写不规范571例,占59.60%;不合理用药387例,占40.40%。结论我院急诊药房处方存在一定问题,临床医师、药师仍需不断提高业务水平,处方书写规范、合理用药等有待加强。

Objective To investigate the analysis of Emergency Pharmacy unqualified prescriptions, which aims to improve prescription quality, promote the rational use of drugs to provide reference. Methods In our hospital in Jan. 2009 - Dec. 2011 prescription were investigated, with＂ prescription management method （try out ）＂ as the basis, analysis of unqualified prescriptions written not standardized and rational drug use. Results Emergency Pharmacy in our hospital in Jan. 2009 - Dec. 2011 22387 prescriptions were, among them unqualified prescriptions of 958, a failure rate of 4.28%. Unqualified reason includes the writing is not standardized in 571 cases, accounting for 59.60%; irrational drug use in 387 cases, accounting for 40.40% of. Conclusion The emergency department of our hospital pharmacy prescription exist certain problems, clinicians, pharmacists still needs to continuously improve the operational level, standard of prescription writing, reasonable use of antibiotics should be strengthened.
