摘要 目的 研究细胞粘附分子 CD44变异型 (CD44 v)在食管癌中的表达 ,探讨 CD44变异型表达与食管癌侵袭转移的关系。方法 采用免疫组织化学 L SAB技术 ,检测 80例人食管癌手术切除标本变异型 CD44 v5、CD44 v6、CD44 v7和CD44 v10蛋白表达 ,分析其表达与癌侵袭转移的关系。结果  CD44 v蛋白表达与患者的年龄、性别、食管癌病理分型无明显关系。在正常食管鳞状上皮 ,CD44 v5、CD44 v6、CD44 v7和 CD44 v10在上皮的基底层细胞及近基底层细胞均见表达 ,浅表层上皮表达减弱或阴性 ;在癌组织中 ,与 CD44 v5、CD44 v6相比 ,CD44 v7蛋白表达有非常显著性差异 (P <0 .0 1) ,CD44 v10蛋白表达有显著性差异 (P<0 .0 5 ) ;在转移癌中 ,CD44 v5、CD44 v6蛋白表达与原发性癌相比无显著差异 ;但 CD44 v7蛋白表达与原发性癌相比有非常显著性差异(P<0 .0 1) ,CD44 v10蛋白表达有显著性差异 (P<0 .0 0 5 )。结论  CD44 v7和 v10蛋白表达阳性率降低与食管癌侵袭转移潜能相关 ,是判断食管癌侵袭转移的标志之一。 Objective To study the expression of the cell adhesion molecule CD44 variant isoforms and its possible role in the development and progression of esophgeal squamous cell carcinoma.Methods Expression of CD44v5、CD44v6、CD44v7 and CD4410 proteins in 80 cases of human esophageal squamous cell carcinoma were examined by the LSAB technique of immunohistochemistry using monoclonal antibodies directed against specific CD44s and CD44 variant domains.The association between CD44v5、CD44v6、CD44v7、CD44v10 expression and the infiltration and metastasis of this carcinoma was analyzed.Results The expression of CD44v protein showed no apparent relationship with the clinicopathological factors including age,sex ,dysplasia,grade of differentiation and degree of invasion.The levels of CD44v5、CD44v6、CD44v7 and CD44v10 expression in esophageal squamous cell carcinoma was down regulated.The average immunoreactivity scores of CD44v7 and CD44v10 were significantly reduced in carcinoma(CD44v7: P <0 01,CD44v10: P <0 05)and in metastatic carcinoma(CD44v7: P <0 01;CD44v10: P <0 05).Conclusion Since CD44v may play an important role in esophageal tumor genesis, down regulation of CD44v7 and CD44v10 may be a useful prognostic indicator.
