补虚泻实针刺法结合卒中单元模式对肩手综合征患者肩部疼痛的影响

Effect of Needling Method of Reinforcing Deficiency and Reducing Excess in Association with Stroke Unit Mode on Shoulder Pain in Patients with Shoulder-hand Syndrome

目的观察补虚泻实针刺法结合卒中单元模式对脑卒中后肩手综合征患者肩部疼痛的影响。方法将80例患者随机分为治疗组和对照组,每组40例。治疗组采用补虚泻实针刺法结合卒中单元模式进行治疗,对照组采用普通针刺法结合卒中单元模式进行治疗,4星期后观察两组视觉模拟评分(VAS)和总体疗效。结果治疗组总有效率为92.5%,优于对照组的87.5%(P<0.05);两组治疗后VAS均较治疗前显著减少(P<0.01),且治疗组治疗后VAS低于对照组(P<0.01)。结论补虚泻实针刺法结合卒中单元模式和普通针刺法结合卒中单元模式均能明显降低脑卒中后肩手综合征患者的肩部疼痛的程度,对脑卒中后肩手综合征有明显的治疗效果,但补虚泻实针刺法结合卒中单元模式的疗效更优一些。

Objective To observe the effect of needling method of reinforcing deficiency and reducing excess in association with stroke unit mode on shoulder pain in patients with post-stroke shoulder-hand syndrome. Method Eighty patients were randomized into a treatment group and a control group, 40 in each. The treatment group was intervened by needling method of reinforcing deficiency and reducing excess in association with stroke unit mode, while the control group was intervened by ordinary needling method in association with stroke unit mode. The score of Visual Analogue Scale （VAS） and treatment effect were observed 4 weeks later. Result The total effective rate was 92.5% in the treatment group, significantly higher than 87.5% in the control group （P〈 0.05）; after treatment, the VAS score dropped significantly in both groups （P〈0.01）, and the VAS score in the treatment group was markedly lower than that in the control group （P〈0.01）. Conclusion Both ordinary needling method and the needling method of reinforcing deficiency and reducing excess in association with stroke unit mode have significant effect in relieving shoulder pain in patients with post-stroke shoulder-hand syndrome, effective in treating shoulder-hand syndrome, while the needling method of reinforcing deficiency and reducing excess in combination with stroke unit mode has certain advantage in treatment effect.
