棍针配合针刺治疗周围性面瘫临床观察

Clinical Observation on Stick Needle and Acupuncture in Treating Peripheral Facial Paralysis

目的观察棍针配合针刺治疗周围性面瘫的临床疗效。方法将60例周围性面瘫患者按照随机数字表分为治疗组和对照组。对照组接受常规针刺治疗,治疗组在常规针刺的基础上配合棍针治疗。记录患者临床疗效、治疗前后的"3等级40分面神经麻痹程度判定评分法"评分和不良反应等的指标,并对观察数据进行分析比较。结果治疗组痊愈率为33.3%,对照组为6.7%,治疗组治愈率高于对照组(P<0.05)。治疗组和对照组总有效率均为96.7%,两组比较差异无统计学意义(P>0.05)。两组患者"3等级40分面神经麻痹程度判定评分法"的各项评分均较治疗前增高(P<0.05);与对照组比较,治疗组吹哨症状评分改善更明显(P<0.05),余结果无统计学差异(P>0.05)。结论与常规针刺治疗比较,棍针配合针刺对周围性面瘫的治愈率更高,可明显改善患者的临床症状,特别是对吹哨症状的改善。

Objective To observe the clinical effect of stick needle together with acupuncture in treating peripheral facial paralysis （FP）. Method Sixty patients with peripheral FP were divided into a treatment group and a control group according to the random number table. The control group received conventional acupuncture, while the treatment group received treatment by stick needle in addition to the conventional acupuncture. The clinical effects, scores of Yanagihara grading system before and after treatment, as well as adverse effects were recorded, analyzed and compared. Result The recovery rate was 33.3% in the treatment group versus 6.7% in the control group, and the recovery rate in the treatment group was significantly superior to that in the control group （P〈 0.05）. The total effective rate was 96.7% in both treatment group and control group, and the difference was insignificant in comparing the improvement rate between the two groups （P〉0.05）. Marked increases of component scores of Yanagihara grading system were found in both groups after treatment （P〈0.05）; a more significant improvement in whistle was found in the treatment group compared with the control group （P〈0.05）, and the differences of other components between the two groups were insignificant （P〉0.05）. Conclusion Compared with conventional acupuncture, combined use of stick needle and acupuncture can achieve a higher recovery rate in treating peripheral FP, significantly improving clinical symptoms especially whistle.
