摘要
【目的】探讨食管癌术后发生并发症的危险因素。【方法】采用病例对照研究的方法回顾胸外科食管癌根治术后患者,将其中出现并发症的120例患者与同期行食管癌手术未出现并发症的患者进行各个危险因素对比,并将各种危险因素进一步行Logistic回归分析。【结果】单因素分析发现下列因素与食管癌术后并发症的发生有显著相关:年龄60岁以上,术前全身情况,基础疾病,术前病理分级,入信ICU时间,手术时间,输血量,激素使用,意识障碍,抗酸制剂的使用,术后纤支镜使用,术后循环情况,液体出入量,术后氧合情况。Logistic回归分析食管癌术后并发症的高度相关的危险因素为:年龄,术前全身情况,术后循环情况,术后氧合情况。【结论】年龄,术前全身情况,术后循环情况,术后氧合情况这些因素可能为食管癌术后出现并发症的高危因素。
[Objective]To explore the risk factors of postoperative complications of esophagus cancer. [Methods] Pa tients with esophagus cancer undergoing radical operation in the department of chest surgery were retrospectively analyzed by using case-control study method. The risk factors between 120 patients with complications and other 120 patients without complications were compared. Logistic regression analysis was used to analyze the risk fctors further. [Results] Univariate analysis showed that the following factors including male, age over 60 years old, preoperative body condition, basic diseases, preoperative pathological grading, days in ICU, operation time, transfusion volume, hormone usage, con- sciousness disorders, antiacid usage, postoperative bronchofiberscope application, postoperative blood circulation, liquid intake and output volume and postoperative oxygenation were significantly related to the incidence of postoperative compli- cations of esophagus cancer. Logistic regression analysis showed that the risk factors closely correlated with postoperative complications of esophagus cancer were age, preoperative body condition, postoperative circulation and oxygenation. [Conclusion] The factors including age, preoperative body condition, postoperative circulation and oxygenation may be the high risk factors of the incidence of the complications after the surgery of esophagus cancer.
