球囊辅助微弹簧圈栓塞颅内宽颈动脉瘤临床分析 被引量:7

摘要 目的探讨球囊辅助瘤颈重塑形技术结合微弹簧圈栓塞治疗颅内宽颈动脉瘤的临床应用价值。方法对行球囊辅助微弹簧圈栓塞的23例颅内宽颈动脉瘤患者的临床资料进行回顾性分析,其中动脉瘤位于颈内动脉后交通动脉段8例,前交通动脉瘤7例,大脑中动脉分叉部6例,基底动脉顶端2例,均应用球囊辅助栓塞。观察栓塞效果和患者预后。结果全部患者均致密栓塞,术中顺利,术后2例大脑中动脉分叉部患者出现一过性缺血症状,经治疗缓解,所有患者随访期均无再出血。复查脑血管造影,未见动脉瘤再通。结论球囊辅助微弹簧圈栓塞技术是治疗颅内宽颈动脉瘤的一种安全有效的方法,可增加动脉瘤的致密栓塞率,并有效减少并发症的发生。 Objective To discuss the feasibility, safety, effectiveness and technique of balloon-assisted endovascular in the treatment of wide-necked aneurysms with mierocoils. Methods The clinical data of 23 patients with wide-necked aneurysms treated by balloon-assisted endovascular embolization with microcoils were analyzed retrospectively. There were 8 patients with posterior communicating artery aneurysms, 7 patients with anterior communicating artery aneurysms, 6 patients with middle cerebral artery bifurcation aneurysms and 2 patients with basilar artery aneurysms. All the aneurysms were embolized with microcoils assisted by balloons. The effect of embolization and the prognosis of patients was observed. Results Twenty-three aneurysms received totally occlusion. The postoperative cerebral transient ischemia occurred in 2 patients; rebleeding did not occur in all the patients. Conclusion Balloon-assisted endovascular embolization with microcoils is a safe and effective method to treat wide-necked aneurysms with enhancing dense-packing rate and reducing the postoperative complications.
