摘要
目的探讨某地区维吾尔族与汉族HBV对拉米夫定和阿德福韦天然耐药情况及基因型分布。方法采用基因芯片对来自某地区的维吾尔族、汉族初次感染未使用核苷(酸)类似物抗病毒治疗的HBV患者101例血清标本进行与HBV耐药相关的rtL180M、rtM204I/V、rtA181T/V和rtN236T的耐药突变检测和基因型分析。结果某地区HBV对拉米夫定和阿德福韦的天然耐药率为9.9%,其中维吾尔族耐药率2.17%,汉族16.36%,维吾尔族人群HBV天然耐药率明显低于汉族,差异有统计学意义(χ2=5.6539,P<0.05);在维吾尔族突变的1例中只检出rtM204I/V突变,而在汉族检测突变的9例中4个常见位点的6种突变都有检出。结论该地区维吾尔族HBV对拉米夫定和阿德福韦的天然耐药发生率较汉族低,且对拉米夫定耐药者多见,而汉族HBV对这两种药物耐药都可检出。
Objective To analyze the resistant genotype distribution and the natural resistance mutation of hepatitis B virus(HBV) in Uyghur and Han Nationality in certain area.Methods 101 serum samples were collected from patients of Uyghur and Han Nationality with primary infection of HBV and didn’t receiving nucleoside(nucleotide) analogs for antiviral treatment,and all of the samples were detected by gene chips for rtL180M,rtM204I/V,rtA181T/V and rtN236T mutations.Results The natural resistance mutation rate of HBV in Uyghur Nationality was 2.17%,significantly lower than 16.36% of Han Nationality(χ2= 5.653 9,P〈0.05).Only one case in Uygur Nationality was detected with resistance mutation of rtM204I/V,and the 4 common sites of 6 kinds of mutations were detected in Han Nationality.Conclusion The natural resistance mutation rate of HBV to lamivudine and adefovir dipivoxol in Uyghur Nationality might be much lower than that in Han Nationality,and the more are lamivudine resistance.But in Han Nationality,the two kinds of drugs resistance to nucleoside(nucleotide) analogs both could be detected.
