摘要
目的观察急性坏死性胰腺炎(ANP)大鼠甲状腺滤泡旁细胞超微结构和功能的变化,探讨ANP并发低钙血症的可能机制。方法雄性Wistar大鼠32只,按数字表法随机分为假手术组(SO组)及ANP3、6、12h组。采用胆胰管逆行注射5%牛磺胆酸钠溶液制备ANP模型。检测血淀粉酶、脂肪酶、降钙素及Ca2+水平。取胰腺组织常规病理检查,取甲状腺腺叶组织电镜下观察滤泡旁细胞超微结构改变。结果ANP组6h的血淀粉酶、脂肪酶、降钙素水平分别为(4025±579)U/L、(8272±794)U/L、(383.6±64.5)pg/ml,均显著高于s0组的(1063±129)U/L、(55±18)U/L、(143.1±42.2)pg/ml;血清Ca2+水平为(2.41±0.12)mmol/L,显著低于SO组的(2.97±0.12)mmol/L(P值〈0.05或〈0.01)。ANP组12h的血清降钙素水平为(317.1±78.2)pg/ml,较6h时显著降低(P〈0.01)。ANP组大鼠甲状腺滤泡旁细胞核膜内陷,线粒体、游离核糖体增生,粗面内质网、高尔基体融合,分泌颗粒增多;6h后细胞器开始减少并消失,仅余少量分泌颗粒。结论ANP大鼠的甲状腺滤泡旁细胞结构和功能发生改变,从而导致低钙血症的发生。
Objective To investigate the uhrastructure and function changes of the thyroid parafollicular cell in ANP rats and to study the possible mechanism of hypocaleemia during ANP. Methods Thirty-two male Wistar rats were randomly divided into 4 groups : sham operation group ( SO group) and ANP 3, 6, 12 h groups. ANP model was induced by retrograde injection of 5% sodium taurocholate into the biliopancreatie duct. The serum levels of amylase, lipoidase, calcitonin and Ca2+ were measured. The pathological changes in pancreatic tissue were observed. The uhrastructure changes of thyroid parafollieular cell were observed. Results The serum levels of amylase, lipoidase, ealcitonin and Ca2+ were (4025 ±579)U/L, (8272 ± 794) U/L, ( 383.6 ± 64.5 ) pg/ml, (2.41 ± 0.12) mmol/L at 6h in ANP group, which were significantly higher than those in SO group [ ( 1063± 129 ) U/L, ( 55 ± 18 ) U/L, ( 143.1± 42.2 ) pg/ml, (2.97 ± 0.12) mmol/L ( P 〈 0.05 ) ]. Under the electron-microscopy, the nucleus of thyroid parafollieular cells shrinked and mitochondrial proliferation and endoplasmic retieulum fusion appeared. After 6 h, all the organelle reduced and disappeared, and only small amount of secretory granules remained. Conclusions Changes in the structure and function of the thyroid parafollieular cells occur in rats of ANP, and they induce the development of hypocaleemia.
