MSCT曲面重建成像在面神经管上的应用价值 被引量:2

摘要 目的:通过多层螺旋CT容积扫描,探讨曲面重建成像对面神经管走行全貌及各段管径大小的显示能力。方法:随机对我院2011年6月至2012年3月行内耳检查并无颞骨病变的45例患者,行多层螺旋CT颞骨岩部高分辨率容积扫描,随后将采集数据进行超薄层原始放大后传到图像后处理工作站,完成双侧面神经管的曲面重建,并分别测量管腔宽径大小及各段长度值。结果:45例患者(90耳)中40例(88.9%)能完整显示双侧面神经管走行,并能准确测量各段管径大小及长度,3例(6.7%)仅显示一侧的面神经管,2例(4.4%)双侧均无法显示。面神经管左、右侧迷路段、鼓室段、乳突段及总长分别为(3.66±0.31)mm、(11.55±1.15)mm、(13.51±1.08)mm、(26.32±2.61)mm和(3.53±0.23)mm、(11.24±1.21)mm、(13.12±1.24)mm、(26.73±2.31)mm。面神经管左、右侧迷路段、鼓室段、乳突段及总管径分别为:(0.82±0.11)mm、(1.15±0.12)mm、(1.18±0.14)mm、(3.09±0.11)mm和(0.79±0.13)mm、(1.19±0.21)mm、(1.21±0.15)mm、(3.15±0.22)mm。结论:多层螺旋CT曲面重建对显示面神经管的影像解剖具有重要的价值,为面神经病变的诊断提供了理论基础。 Objective:To apply MSCT volume scanning and explore the displaying ability of CPR (curved planar reformation) imaging technique of MSCT in the panorama of facial nerve canal and the size and the diameter of each section. Methods:Forty-five cases,who were administered inner ear examinations between June 2011 and March 2012 and found no abnormality in temperal bone ,were randomly selected to have high resolution MSCT volume scanning on the part of temperal bone. All the acquisition data were processed by the means of ultra-thin layer raw data magnification and sent to the image post-processing worker-station to complete the curved planar reformation of the bilateral fa- cial nerve canals measuring the size,the diameter,and length of each section. Result:Among the 45 cases,89. 90 % of them (40 cases,90 ears) could completely display the bilateral facial nerve canals and the size, the diameter, and length of each section could be accurately measured. Three cases (6. 7% ) could only display the unilateral facial nerve canal ,and two cases (4. 4% ) displayed none of the bilateral fa- cial nerve canals. The length of labyrinthine segment,tympanic segment,and mastoid segment of the facial nerve canals had a total length on the left and right of (3.66 ±0.31) mm,(11.55 ± 1.15) mm, (13.51 ± 1.08) mm,(26.32 ±2.61) mm,and (3.53 ±0.23) mtn,(ll.24 ± 1.21 ) mm, (13. 12 ± 1.24) mm, (26. 73 ±2. 31 ) mm,respectively. The size and the diameter of the labyrinthine segment,tympanic segment and mastoid segment of the facial nerve canals had a total length on the left and right of (0. 82 ±0. 11 ) mm, ( 1.15 ± 0. 12) mm, ( 1.18 ± 0. 14) mm,(3.09 ±0. 11) mm,(0.79 ±0. 13) mm,(1.23 ±0. 15) mm,(1.21 ±0. 15) mm,and (3. 15 ±0.22) mm,respectively. Conclu- sion:The CPR imaging technique of MSCT has a significant value in the display of the imaging anatomy of facial nerve canal and provides a theoretical foundation for the confirmation position diagnosis of lesions of the facial nerve.
