温阳益气、活血利水法对慢性心衰患者NT-proBNP和心功能的影响研究

Effect of Methods of Warming Yang and Benefiting Qi and Promoting Blood Circulation and Inducing Diuresis on Serum NT-proBNP Level and Cardiac Function in Chronic Heart Failure

目的:观察温阳益气、活血利水法对慢性心力衰竭患者心功能及NT-proBNP的影响。方法:将68例患者按随机数字表法分为两组。其中对照组30例接受常规治疗;治疗组38例在常规治疗的基础上加用温阳益气、活血利水法中药。两组均以4周作为观察期。观察两组患者治疗前后临床疗效及心功能分级,超声心动图参数左室收缩末期内径(LVESD)、左室收缩末期容积(LVESV)、左室射血分数(LVEF),血浆NT-proBNP水平等的变化;并观察药物的安全性、不良反应。结果:心功能分级疗效治疗组总有效率为92.1%,对照组为70.0%,两组比较差异有统计学意义(P<0.05)。两组治疗后超声心动图各参数(LVESD、LVESV、LVEF)和血浆NT-proB-NP水平均较治疗前明显改善(P<0.05或P<0.01),且治疗组较对照组改善更显著(P<0.05或P<0.01)。结论:温阳益气、活血利水法治疗慢性心力衰竭疗效确切,可通过降低血浆NT-proBNP水平,改善心功能。

Objective ： To investigate the effects of methods of warming Yang and benefiting Qi and promoting blood cir- culation and inducing diuresis（ TCM treatment）on serum NT -proBNP level and cardiac function in patients with chronic heart failure（CHF）. Methods ：Sixty- eight patients with CHF were chosen and divided randomly into two groups ：control group（ n = 30）was treated with routine treatment and treatment group（n = 38 ）treated with TCM treatment and routine treatment. Effects in both groups were observed for 4 weeks. The clinical effect, cardiac function, serum levels of LVESD, LVESV, LVEF and NT - proBNP were studied. Results ~ The total effective rate in treatment group （92.1% ） was better than that in the control group（70.0% ） （P 〈 0.05 ）. After treatment, LVESD, LVESV, LVEF and NT - proBNP se- rum levels of two groups were improved（P 〈0.05 or P 〈0.01 ）. And the LVESD,LVESV,LVEF and NT - proBNP ser- um levels in treatment group were improved significantly compared with those in control group （P 〈 0.05 or P 〈 0.01 ）, Conclusion ： The TCM treatment of warming Yang and benefiting Qi and promoting blood circulation and inducing diuresis has good effect on treating CHF,which could decrease NT- proBNP serum level and improve cardiac function.
