MPCNL治疗肾、输尿管上段结石22例体会

CLINICAL EXPERIENCE OF TREATING 22 CASES RENAL CALCULI AND UPPER URETERAL CALCULI WITH MINIMALLY INVASIVE PERCUTANEOUS NEPHROLITHOTOMY

目的:评价微通道经皮肾钬激光碎石术(MPCNL)治疗肾、输尿管上段结石的疗效。方法:22例肾结石、输尿管上段结石患者,在B超和X线C臂机定位引导下,建立微创工作通道行经皮肾一期钬激光碎石。结果:1例患者术中出血,终止手术置造瘘管后转上级医院行二期PCNL,其他患者均准确建立理想的穿刺、工作通道,均无明显结石残留。结论:微通道B超和X线引导下经皮肾钬激光碎石术治疗肾结石和输尿管上段结石,既可提高工作通道建立的准确性及安全性,增加手术的成功率,而且术中、术后并发症较少,是临床治疗肾、输尿管结石的理想方法。

Objective：To evaluate the therapeutic effects of treating renal calculi and upper ureteral calculi with minimally invasive percutraneous nephrolithotomy （MPCNL）.Methods：22 cases renal calculi and upper ureteml calculi patients were treated with one-stage MPCNL under guidance of B ultrasound combined C-model arm X-rays machine. Results：Precise operative tract was successfully established in all cases, except one case went to better hospital for two-stage MPCNL because of bleeding during operation. No case suffered residual stones and severe complications. Conelusions：MPCNL under guidance of B ultrasound combined radiography can improve the precision and safety in establishing operative tract, increase successful rate of operation, and can decrease complications. So MPCNL is the ideal method to treat renal calculi and ureteral calculi.
