急性脑出血患者白细胞计数与肌钙蛋白Ⅰ及心律失常的相关性 被引量:3

Relationship of White Blood Cell Counts with Cardiac Troponin I and Arrhythmia in Patients with Acute Cerebral Hemorrhage

摘要 目的观察急性脑出血(acute cerebral hemorrhage,ACH)患者急性期血白细胞计数与心律失常的相互关系,探讨白细胞计数对ACH患者心律失常发生的预测价值。方法观察未行手术治疗的115例ACH患者,根据血白细胞计数分为3组:A1组(<10.0×109/L)35例,A2组(10.0×109~15.0×109/L)43例,A3组(>15.0×109/L)37例,测定各组患者心肌肌钙蛋白I(cardiac troponin I,cTnI)、肌酸激酶MB型同工酶(creatine kinase-MB,CK-MB)水平,统计各组患者心律失常的发生率,分析相关性。结果 3组患者cTnI及CK-MB水平、心律失常的发生率比较,差异有统计学意义(P<0.01),A2、A3组显著高于A1(P<0.01)。血白细胞计数与心律失常发生率呈正相关(r=0.869,P<0.01)。结论 ACH患者血白细胞计数越高,肌钙蛋白含量越高,心律失常特别是恶性心律失常的发生率越高。 Objective To observe the relationships of white blood cell(WBC) counts with cardiac troponin I(cTnI) and arrhythmia in patients with acute cerebral hemorrhage(ACH),and probe the predictive value of WBC counts in the occurrence of arrhythmia.Methods A total of 115 ACH patients without surgery were divided into 3 groups according to the WBC counts: group A110.0×10^9/L(35 cases),group A2 10.0×10^9-15.0×10^9/L(43 cases),grorp A315.0×10^9/L(37 cases).CTnI and creatine kinase-MB(CK-MB) in each group were measured,the incidence rate of arrhythmia was counted,and their correlation was analysed.Results The concentrations of cTnI and CK-MB,and the incidence rates of arrhythmia were significant different among three groups(P〈0.01),and higher in groups A2 and A3 than those in A1 group(P〈0.01).WBC counts was positively correlated with the incidence rate of arrhythmia(r=0.957,P〈0.01).Conclusion The higher the level of WBC counts in ACH patients,the higher the cTnI content and the incidence rate of arrhythmia,especially malignant arrhythmia.
