直肠癌术后并发直肠阴道瘘的围手术期护理研究

The perioperative nursing after resection of rectal cancer complicated with rectovaginal fistula

目的探讨直肠癌术后并发直肠阴道瘘围手术期全程优质护理的临床效果。方法回顾性分析2009年1月-2011年1月我院收治的14例直肠癌术后并发直肠阴道瘘的患者治疗护理过程。通过围手术期全程优质护理，在术前、术后密切观察病情，监测心肺功能，监控引流管通畅，鼓励患者早期下床活动。通过调查护理前后患者的生活质量评分表（FACT），观察护理的效果。结果14例患者均治愈。瘘愈合后无复发；护理前生活质量评分（101．43±22．45）分与护理后生活质量评分（155．64±32．54）分比较差异显著，有统计学意义（P〈0．05）。结论直肠癌术后并发直肠阴道瘘手术后采用正确的饮食护理、会阴部护理、心理护理、术前术后护理等措施，有利于患者生活质量的提高。

Objective To study after resection of rectal cancer complicated with rectovaginal fistula peri - operation period Full of quality nursing. Methods From January 2009 to January 2011, the therapy course of 14 cases of rectal cancer postoperative complicated with rectovaginal fistula treatment were analyzed. Through the perioperative period full quality care, close observation, monitoring cardiopul- monary function and the drainage tube patency, the patients were encouraged early ambulation . The patients ＂ quality of life score sheet （FACT） were investigated. The nursing effect was observed. Results All 14 patients were cured. The fistula healed without recurrence; be- fore nursing quality of life score （ 101.43 ± 22.45 ） with after care quality of life score （ 155.64 ± 32. 54） were significantly different （ P 〈 0. 05 ） . Conclusions Postoperative rectal cancer patients complicated with rectovaginal fistula after operation with proper diet nursing, per- ineal nursing, psychological nursing, preoperative and postoperative nursing measures could improve quality of life.
