活动平板运动试验对冠心病诊断价值的临床研究

摘要 目的:将活动平板运动试验(TET)的结果与冠状动脉造影(CAG)的结果对照,探讨活动平板运动试验对冠心病的诊断价值。方法:对96例具有冠心病的患者进行临床的诊断分析,1周内通过医学的造影观察和平板运动的检查进行分析。统计分析活动平板运动试验的敏感性、特异性,比较对不同程度冠状动脉病变的敏感性。结果:平板运动试验检出冠心病敏感性80.4%,特异性75.5%,准确率78.0%,其敏感性与冠状动脉的病变程度有关,冠脉多支病变的敏感性高于单支病变(P<0.05)。结论:活动平板运动试验是一项诊断冠心病较理想的非创伤性的检查方法,尤其对于多支血管病变患者,全面综合分析可提高诊断准确率。 Objective: To determine the diagnostic value of treadmill exercise test(TET) in patients with coronary heart disease(CHD) by comparing the diagnostic conclusion with coronary angiography(CAG).Methods: 96 patients with CHD or suspected CHD underwent TET and coronary angiography(CAG) within 1 week.Statistical analyses were used to assess the sensitity and specificity of TET.The sensivity of TET in the diffrerence of severity of coronary lesion were analyzed.The multi-parameters of TET and severity of coronary lesion were also analyzed to explore the relationship between them.Results: TET identified patients with coronary heart disease with a sensitivity of 80.4%,specificity of 75.5% and accuracy of 78.0%.The sensitivity was associated with the extent of coronary artery disease,In patients with multi-vessel disease were significantly higher than that in patients withsingle vessel disease(P < 0.05).Conclusion: TET is a reliable method in the non invasive method to diagnosis of CHD,especially for patients with multi-vessel diseased.Aggregate analysis acould help improve the diagnostic accuracy of TET.
