社区精神卫生服务与家庭干预对慢性精神分裂症患者康复的作用

Community mental health services and family intervention on the role of rehabilitation of patients with chronic schizophrenia

目的探讨精神卫生服务与家庭干预对慢性精神分裂症患者康复的作用。方法于2010年10月至2011年12月选择本院辖区内的慢性精神分裂症患者68例,随机分为对照组与研究组各34例,对照组患者给予正常的精神病药物治疗,维持原剂量及随访等;研究组患者在常规治疗的基础上给予社区精神卫生服务及家庭干预,即由精神科医师对患者及家属进行健康教育,提供咨询指导,症状判断及康复技巧等,并由精神科医生每周1次进行家庭干预。半年后对两组患者采用Morning Side康复状态量表(MRSS)及生活质量量表评价患者康复情况及生活质量改善情况。结果两组干预前基本资料比较差异无统计学意义(P>0.05),干预后研究组MRSS总分及各项目评分较干预前明显降低,前后差异有统计学意义(P<0.05);对照组MRSS总分及各项目评分较之前无明显变化(P>0.05),两组干预后比较差异有统计学意义(P<0.05);生活质量方面,干预半年后研究组患者的工作能力、独立性、躯体健康、生活自理能力以及自我评估和人际关系均优于对照组(P<0.01)。结论在常规药物治疗的基础上给予社区精神卫生服务及家庭干预对慢性精神分裂症患者康复及生活质量提高有明显的积极作用。

Objective To investigate the mental health services and family intervention on the role of rehabilitation of patients with chronic schizophrenia.Methods In October 2010 December 2011 within the jurisdiction of the Court selected 68 patients with chronic schizophrenia, stochastic analysis of the control group and study group,34 cases of normal control group were treated with psychiatric medications, to maintain the original dose and follow-up, etc., study group patients in the conventional treatment given on the basis of community mental health services and family intervention, that is, from psychiatrists to patients and their families, health education, provision of counseling, and rehabilitation techniques to determine the symptoms, a doctor by the spirit once a week for family intervention. Six months later the two groups of patients with Morning Side Rehabilitation Status Scale （MRSS） and evaluation of quality of life scale rehabilitation of patients with improvement in the situation and quality of life. Results There were more basic information before the intervention was no significant difference （P〉 0.05）, after the intervention and the project team MRSS score was significantly lower score than the intervention, before and after difference was statistically significant （P 〈0.05）, control group MRSSMRSS total score and the score than before the project was no significant change （P〉 0.05）, after the intervention groups was statistically significant difference （P〈0.05）; quality of life, intervention studies in patients six months after the ability to work independently sexual, physical health, self-care ability and self- evaluation and interpersonal relationships than the control group （P〈O.O1）. Conclusion In the conventional therapy given on the basis of community mental health services and family intervention on the rehabilitation of patients with chronic schizophrenia and quality of life improved significantly positive effect.
