妊娠期糖尿病患者血尿酸与胰岛素抵抗的关系

The clinical study on the relationship between blood uric acid and insulin resistance in the patients with gesta- tional diabetes mellitus （ GDM ）

目的探讨妊娠期糖尿病（GDM）患者血尿酸水平与胰岛素抵抗的相关性。方法检测80例GDM患者与40例正常妊娠产妇的血尿酸（uA）、空腹血糖（FPG）、空腹胰岛素（FINS）、胰岛素抵抗指数（IRI）等，并将GDM患者按血尿酸水平分为A组（高尿酸血症，血尿酸〉360Ixmol／L）31例和B组（血尿酸正常）49例，观察比较各组间观察指标变化。结果GDM组的FPG、UA、FINS、IRl分别为（7．87±0．69）mmol／L、（308．874-79．12）txmol／L、（15．87±5．12）mU／L、（2．27±0．72），与对照组的（4．57±1．12）mmol／L、（284．57±43．12）μmol／L、（12．57±4．12）mU／L、（1．32±0．62）相比较，差异均有统计学意义（t=17．0842、2．1758、3．5410、7．1247，均P〈0．05）。A组FPG、UA、FINS、IRI分别为（8．38±0．46）mmol／L、（373．97±29．12）txmol／L、（17．32±6．12）mU／L、（2．47±0．12），与B组的（7．644-0．46）mmoL／L、（284．57±33．12）ixmol／L、（14．57±3．12）mU／L、（2．04±0．22）相比较，差异均有统计学意义（t=5．7187、12．3116、2．3185、11．2835，均P〈0．05）。GDM患者血尿酸水平与FINS、IRI、TG呈明显的正相关关系（r=0．839、0．720、0．660，均P〈0．05）。结论GDM患者血尿酸水平与胰岛素抵抗具有相关性。

Objective To investigate the relationship between blood uric acid and insulin resistance in the patients with GDM. Methods The various biochemical parameters such as uric acid （ UA）, fasting plasma glucose （FPG） ,fasting insulin（FINS） and insulin resistance index（IRI） were detected in 80 patients with GDM and 40 nor- mal pregnant women. 80 patients with GDM were divided into A group （ hyperuricemia, UA 〉 360μmol/L, 31 cases ） and B group （normouricemia, 49 cases ） to observe and compare the changes of each biochemical parameter. Results Compared to the control group, there were significant differences in FPG [ （ 7.87 ± 0.69 ） mmol/L, t = 17. 0842] ,UA [ （ 308. 87 ± 79. 12 ） μmol/L, t = 2. 1758 ], FINS [ （ 15.87 ± 5. 12 ） mU/L, t = 3. 5410 ] and IRI [ （2.27 ±0.72）,t =7. 1247] （all P 〈0.05）. There were significant differences in FPG（8.38 ±0.46） mmol/L,TG （373.97 ±29.12）μmol/L,FINS（ 15.87± 5. 12）mU/L, IRI（2.47 ±0. 12） between group A and group B（t = 5. 7187,12. 3116,2. 3185,11. 2835, all P 〈 0.05 ）. The blood uric acid level was positively related to FINS, IRI, TG in the patients with GDM （ r = 0. 839,0. 720,0. 660, all P 〈 0.05 ）. Conclusion There was correlation between the blood uric acid level and insulin resistance in the patients with GDM.
