新型农村合作医疗不同参合人群住院受益分析

Benefit of inpatients in different economic groups in the new rural cooperative medical scheme(NRCMS)

目的:探讨不同经济收入水平参合人群住院受益公平性。方法:将参合对象划分为五保户、低保户、一般参合对象3个类别,利用肥西县2011年全年新农合住院补偿数据,分析3类人群住院服务利用情况及新农合补偿受益情况。结果:3类不同经济收入水平人群的住院受益率、住院流向、实际补偿比及基金使用率均存在差异,相对最贫困的五保户的住院受益率、实际补偿比及基金使用率最高。结论:新型农村合作医疗住院受益较为公平,这于新农合政策向贫困群体倾斜有关。同时存在贫困群体对高层次卫生服务的利用相对较少及其他参合人群就诊流向不合理的问题。

Objective To evaluate benefit equity of inpatients in different economic groups in the new rural coopera tive medical scheme （NRCMS）. Methods The rural residents included the five guarantees family,extremely low income households,and the general residents. By calculating the government subsidy in NRCMS among different economic groups,we evaluated the inpatient service and the benefit equity in different groups in Feixi in 2011. Results There were differences in the benefit ratio of inpatient,the actual compensation ratio and the fund utilization ratio among inpatients of different economic levels. Five guarantees family benefited most among all groups. Conclusion Inpatient subsidy bene fit of the NRCMS is fair,in which the policy favors the poor community. There still exist some problems including the less utilization of high level health service among poor population and irrational distribution of medical consultation.
