痰火型特发性真性性早熟女孩心理行为及中医药治疗临床观察

Analysis of Psychological Behavior of Idiopathic Central Precocious Puberty Girls with Phlegm-fire Type and Efficacy of Chinese Medicine

目的:观察痰火型特发性真性性早熟(ICPP)女孩的心理行为状况,并评估中医药治疗疗效。方法:45例ICPP患儿予清热化痰法中药治疗6月,治疗前后采用阿成贝切儿童行为量表(CBCL)进行心理行为评估,同时比较治疗前后中医证候总分及卵巢容积、预测身高。40例同期健康体检女童设为对照组。结果:CBCL的多动、攻击、违纪分值,治疗组治疗前较对照组高,2组比较,差异均有显著性意义(P<0.05)。治疗后治疗组患儿的以上分值低于治疗前,攻击、违纪分值差异均有显著性意义(P<0.05)。多动分值差异无显著性意义(P>0.05)。治疗组治疗后卵巢容积小于治疗前,差异有显著性意义(P<0.05),预测身高高于治疗前,但差异无显著性意义(P>0.05),中医证候总分治疗后低于治疗前,差异有非常显著性意义(P<0.01)。结论:痰火型ICPP患儿存在一定程度的多动、攻击、违纪的不良情绪和行为,中医药治疗能缓解该证型患儿的部分不良情绪,改善临床症状,疗效确定。

Objective： To observe the psychological behavior of idiopathic central precocious puberty （ICPP） girls with phlegm-fire type, and to assess the efficacy of Chinese medicine. Methods： Forty-five qualified patients were treated with Chinese medicine with the actions of clearing heat and resolving phlegm for 6 months. Their psychological behavior was assessed by Achenbach＇ s child behavior check list（CBCL） before and after treatment, and the total score of traditional Chinese medical syndrome, ovarian volume and prediction of height were compared before and after treatment. Forty healthy girts who received medical examination at the same time served as the heatthy control. Results= The CBCL scores in hyperactivity, attack and discipline of treatment group were higher than those of the healthy control, and the difference was significant （P 〈 0.05）. After treatment, the attack score and discipline score in the treatment group were significantly lower than those before treatment, and the difference was significant （P 〈 0.05）. However, the difference of hyperactivity score in the treatment group was insignificant before and after treatment （P 〉 0.05）. Ovarian volume in ICPP girls after treatment was significantly smaller than that before treatment, and the difference was significant （P 〈 0.05）. The total score of traditional Chinese medical syndrome after treatment was significantly lower than that before treatment in the treatment group, and the difference was significant （P 〈 0.05）. Predicted of height in ICPP girls after treatment was higher than before, but the difference was insignificant （P 〉 0.05）. Conclusion： The phlegm-fire type ICPP girls have certain degrees of hyperactivity, attack and discipline. Chinese medicine therapy can help the girls to ease some of bad mood and relieve the clinical symptoms, which has certain curative effect.
