针灸分期治疗促进胫骨中下段骨折愈合的疗效观察

Clinical Observation on the Effect of Promoting Middle and Distal Tibial Fracture Healing Curative Effect by Treatment of Acupuncture and Moxibustion for Different Stages

目的:观察针灸分期治疗促进胫骨中下段骨折愈合的疗效。方法:将90例符合纳入标准的本病患者随机分为针灸分期组、常规针刺组、药物组,每组30例。3组均常规应用抗生素抗感染治疗,均治疗56 d,观察3组疼痛、肿胀度指标,评价其综合疗效,并对愈合时间进行比较。结果:3组患者肿胀疗效比较针灸分期组优于常规针刺组、药物组(P<0.05)。患者治疗3 d后、7 d后VAS评分均较治疗前明显降低(P<0.001),针灸分期组与同时间点常规针刺组、药物组比较降低更显著(P<0.01),常规针刺组与药物组同时间点比较,差异亦有统计学意义(P<0.001),针灸分期组在镇痛方面优于常规针刺组和药物组,而常规针刺组优于药物组。3组总有效率均为100%,但3组综合疗效比较,差异有统计学意义(P<0.05),针灸分期组优于常规针刺组和药物组。3组治愈病例骨折愈合时间比较,差异有统计学意义(P<0.001),针灸分期组短于常规针刺组和药物组,差异有统计学意义(P<0.01)。结论:针灸分期治疗可明显改善胫骨中下段骨折患者肿胀、疼痛等临床症状及关节活动度,缩短骨折愈合时间,综合疗效显著,是一种安全、有效的促进骨折愈合的方法。

Objective： To observe the effect of acupuncture and moxibustion for different stages of promoting middle and distal tibial fracture healing. Methods： 90 patients were randomly divided into acupuncture and moxibustion staging treatment group, routine acupuncture group and drug group, 30 cases in each group. The three groups applied the routine treatment of antibiotics to prevent infection for 56 days. The pain level, swelling index was observed to evaluate the clinical effect, and the healing time were compared. Results： In the curative effect of swelling, acupuncture and moxibustion staging treatment group is better than that of routine acupuncture group and drug group（P〈0.05）. After 3 and 7 days Treatment, the VAS score were significantly lower than before（P〈0.01）; acupuncture and moxibustion staging treatment group was significantly lower than the routine acupuncture group and drug group at the same time（P〈0.01）; there was also significant difference （P〈0.001） be- tween the routine acupuncture group and drug group at the same time ; in the analgesic effect, acupuncture and moxibustion staging treatment group is better than that of routine acupuncture and drug group, while the routine acupuncture group is bet- ter than that of drug group. The total effective rate of 3 groups were 100%, but there was significant difference（P〈0.05） in the integrated effect of the three groups had; the integrated effect of acupuncture and moxibustion staging treatment group is better than that of routine acupuncture and drug group. In healing time, acupuncture and moxibustion staging treatment group was shorter than that of routine acupuncture group and drug group, there was significant difference（P〈0.001）. Conclusion：Treat- ment of acupuncture and moxibustion for different stages can obviously improve the clinical symptoms such as swelling, pain and range of motion and shorten the healing time of middle and distal tibial fractures, with remarkable clinical effect; it is a kind of safe and effective treatment in promoting fracture healing.
