阴式剖宫产术后瘢痕妊娠切除修补术的临床应用

Clinical application of vaginal excision and repair of cesarean scar pregnancy

目的探讨阴式剖宫产术后瘢痕妊娠切除修补术的临床应用效果。方法选取2011年2月～2012年2月采用阴式病灶切除加修补术进行治疗的12例剖宫产术后瘢痕妊娠患者为A组,同期进行开腹手术的12例患者为B组,将两组患者的出血量、手术时间、住院时间、血清β-HCG降至正常时间、并发症发生率及术前、术后3d的WHO-QOL-100评分进行比较。结果 A组在出血量、手术时间、住院时间、血清β-HCG降至正常时间、并发症发生率方面均优于B组,术后3d的WHOQOL-100评分也高于B组,差异均有统计学意义(P<0.05)。结论阴式剖宫产术后瘢痕妊娠切除修补术临床效果要优于开腹手术,综合优势更为突出。

Objective To study the clinical application effect of vaginal excision and repair of cesarean scar pregnancy. Methods Twelve patients with cesarean scar pregnancy who were treated with vaginal excision and repair from February 2011 to February 2012 were selected as group A, and 12 patients who were treated with open operation at the same time were selected as group B, then the blood loss, operation time, hospitalization time, time of serum β-HCG returning to the normal, incidence of complications and WHOQOL-100 score before and after the operation at third day of two groups were compared. Results The blood loss, operation time, hospitalization time, time of serum β-HCG returning to the normal, incidence of complications of group A were all better than those of group B, WHOQOL-100 score after the operation at third day was all higher than that of group B, these items of two groups had significant differences （P 〈 0.05）. Conclusion The clinical effect of vaginal excision and repair of cesarean scar pregnancy is better than that of open operation, the comprehensive advantage is obvious.
