孕妇血清人巨细胞病毒脱氧核糖核酸与先天感染的关系

Relationship Between Positive Human Cytomegalovirus DNA in Sera of Pregnant Women and Congenital Cytomegalovirus Infection

目的 :探讨孕妇血清人巨细胞病毒脱氧核糖核酸 (HCMV- DNA)与先天感染的关系。方法 :应用套式聚合酶链反应检测 96例孕妇血清及 32例孕妇分娩时的脐血清 HCMV- DNA。结果 :96例孕妇血清 HCMV- DNA阳性检出率为 10 .4% ;7例血清 HCMV- DNA阳性孕妇脐血清 HCMV - DNA检出率为 5 7% (4 /7) ,2 5例阴性孕妇脐血清 HCMV- DNA检出率为 12 % (3/2 5 ) ,血清 HCMV- DNA阳性孕妇组的 HCMV先天感染率为 5 7% ,明显高于阴性组的 12 % (P <0 .0 5 )。结论 :血清 HCMV- DNA是活动性 HCMV感染的标志。血清 HCMV- DNA阳性的孕妇发生 HCMV先天感染的相对危险度为 4.75。

Objective:To study the relationship between positive Human Cytomegalovirus (HCMV) DNA in sera of pregnant women and that in congenital HCMV infected patients. Methods:The nested polymerase chain reaction (Nest-PCR) was used for detection of HCMV-DNA in sera of pregnant women, placenta, cord blood and amniotic fluid. Results:The positive rate of HCMV-DNA in sera of 96 pregnant women was 10.4%. Among 7 pregnant women with positive HCMV-DNA, the positive rate of HCMV-DNA in cord blood, placenta and amniotic fluid were 57% (4/7), 85.7%(6/7)and 75% (3/4), respectively. Among 25 pregnant women with negative HCMV-DNA, the positive rates of the same specimens were 12% (3/25), 16%(4/25)and 10% (1/10), respectively. In HCMV-DNA positive group, the rate of congenital HCMV infection patients was 57%, which was significantly higher than that in control group (12%) (P<0.05).Conclusion:The HCMV-DNA in serum is a mark of active HCMV infection. The relative risk of congenital HCMV infection among the pregnant women with positive HCMV-DNA in sera is 4.75.
