摘要
目的观察用组织扩张器延长损伤后变性的周围神经远段作为修复神经缺损的实验结果。方法以容积为 12 ml外形近似长方体的组织扩张器,3周内缓慢延长 15只 SD大鼠一侧钳夹并结扎损伤的坐骨神经远段,然后切去延长部分,使两神经断端在无张力下缝合,术后14周,进行电生理测定和组织学观察,并与对侧神经缺损后在张力下缝合作比较。结果用组织扩张器延长的损伤后坐骨神经远段,可延长36.5%。再生神经纤维可通过变性后延长的神经段并获得功能恢复,传导速度为18. 6 m/s,比对照侧 16. 3 m/s略优。结论用组织扩张器延长损伤的周围神经干,可克服神经缺损,利于修复。
Objective This study was designed to evaluate the responses of degenerative peripheral nerves to tissue expansion and the functional recoveries of the nerves after repairment. Methods The 12 ml silastic expanders were placed beneath the injured right sciatic nerves of 15 SD rats and fully inflated for 3 weeks, then the elongated nerves were exposed, examined and repaired after cutting of the injured parts by means of crushing and tighting' At the sametime, the left sciatic nerves were sutured under the tension by removing 0. 5 cm segment as controls. 14 weeks later , the nerve conduction velocity(NCV), gastrocnemius contraction force(GCF) and muscle weight(GMW) on both sides of surviring animals were determined. Results The right sciatic nerves were found to increases 36. 45% in length after removal of the expanders. Nerve regeneration occurred in the nerve trucks on both sides, and the NCVs in the elongated nerves were much better than in the controls. Conclusions Nerve a-cons can regenerate and grow into elongated degeneration peripheral nerve with functional recovery. Nerve conduction velocity relealed better regeneration in nerve expansion group.
