胃肠手术快速康复外科联合微波优化设计方案的临床应用 被引量:7

摘要 目的探讨快速康复外科(FTS)治疗联合微波治疗在胃肠道手术应用的安全性及可行性。方法施行胃肠道手术患者274例,按随机数字表法分为快速康复外科组(A组)98例,快速康复外科联合微波照射治疗组(B组)96例,传统手术组(C组)80例,比较3组患者各项临床指标。结果 3组患者术后吻合口漏、重置胃肠减压管比率、手术时间、术中出血量、治疗费用比较差异无统计学意义(P>0.05);B组切口感染、肺部感染、尿路感染、恶心呕吐发生率低于A组、C组(P<0.01);B组术后恢复肛门排气时间、切口愈合时间、术后住院时间均明显短于A组、C组(P<0.01)。结论快速康复外科联合微波照射治疗应用于胃肠道手术,有利于患者的术后康复,并可降低术后并发症,是一种安全、可行的治疗方法。 Objective To study the safety and feasibility of the optimized design scheme of fast-track surgical treatment in gastrointestinal surgery. Methods Two hundred and seventy-four patients undergoing gastrointestinal surgery were randomly divided into three groups :98 cases underwent fast-track surgery( Group A) ,96 cases underwent microwave irradiation combined with fast-track surgery ( Group B ), 80 cases underwent traditional operation ( Group C ). A comparison of clinical indices was done among the three groups. Results There were no significant differences in the complication rate of postoperative anastomotic leakage and gastrointestinal decompression tube reset ,operation duration, blood loss during operation and treatment cost among the three groups (P 〉 0.05 ). The incidences of incision infection, lung infection, urinary tract infection, nausea and vomiting in Group B were significantly lower than those in Group A, Group C ( P 〉 0.05 ) ; The restoration time of anus exhaust, incision healing duration and postoperative hospital stay in Group B were shorter than those in Group A, Group C (P 〈 0.01 ). Conclusion The application of fast-track surgery combined with microwave irradiation to gastrointestinal surgery may benefit patients' postoperative recovery and reducing postoperative complications,which is a safe and feasible therapy.
