摘要
目的探讨产前筛查对单脐动脉的诊断价值和临床意义。方法回顾性分析2009年10月-2011年10月我院产前超声筛查发现的124例单脐动脉临床和超声资料。结果超声诊断胎儿单脐动脉124例,单纯性单脐动脉101例,合并其他结构畸形23例,其中合并心血管系统畸形13例,泌尿系畸形4例,中枢神经系统畸形4例,四肢畸形3例,唇裂或唇腭裂3例,消化道畸形2例,前腹壁畸形1例,羊水异常24例(羊水过多7例,羊水过少17例),合并其他畸形1例。超声动态监测胎儿宫内生长发育情况,9例发生胎儿生长受限(FGR),2例胎死宫内,2例双胎输血综合征。33例行染色体核型分析,发现染色体异常2例。结论超声检查发平均临产时间现胎儿单脐动脉(SUA)后,应仔细检查有无合并其他器官组织结构畸形,并建议行染色体核型分析,以减少出生缺陷。超声动态监测胎儿生长发育情况,加强SUA胎儿产后的检查及随访。
Objective To explore the clinical value of prenatal uhrasonography screening for detecting single umbilical artery (SUA). Methods The data of 124 cases with SUA diagnosed by prenatal ultrasonography between October 2009 and October 2011 was retrospectively analyzed. Results Of the 124 cases of SUA diagnosed by uhrasonography, 101 eases were simple SUA, and the rest complicated with malformations. Thirteen cases had cardiovascular anomaly, 4 with urinary system anomaly, 4 with central nervous system anomaly, 3 with extremity anomaly, 3 with cheilognathus or cheilopalatognathus, 2 with alimentary tract anomaly, 1 with abdominal wall anomaly, 24 with amniotic fluid anomaly (polyhydramnios in 7 cases, oligohydramnios in 17 cases) , and 1 case had other malformation. Intrauterine growth of fetus was monitored by dynamic uhrasonograpby, which indicated that 9 cases suffered from fetal growth restriction (FGR) , 2 died in uterus, 2 cases suffered from twin-twin transfusion syndrome (Trs). Of 33 cases that had chromosome analysis, chromosome abnormalities were found in two. Conclusion If SUA is found by uhrasonograpy, it is necessary to check carefully whether the fetus has structure abnormality, and chromosome analysis is recommended to reduce birth defect. Intrauterine growth of fetus should be monitored by dynamic uhrasonography, and postnatal examination and follow-up should be strengthened for fetus with SUA.
