地诺前列酮与缩宫素促宫颈成熟的临床比较

Clinical comparison of dinoprostone with oxytocin for cervical ripenin~

目的探讨地诺前列酮及缩宫素用于晚期妊娠促宫颈成熟的有效性和安全性。方法随机将80例妊娠晚期、无阴道分娩禁忌、单胎头位、有引产指征的待产妇，分为研究组40例，阴道放置地诺前列酮；对照组40例，静滴0．5％的缩宫素。结果用药12小时后，研究组宫颈成熟度明显优于对照组（χ^2=20．192，P〈0．01），平均临产时间明显短于对照组（t=3．794，P〈0．01）；研究组24小时分娩率达57．5％，高于对照组的10．0％（χ^2=5．764，P〈0．05）。研究组的引产成功率达82．5％，明显高于对照组的20．0％（χ^2=19．431，P〈0．01）。两组的分娩总产程时间、新生儿结局均无显著性差异（均P〉0．05）。结论地诺前列酮是一种安全、有效促宫颈成熟和引产的药物。

Objective To explore the validity and safety of dinoprostone and oxytocin for cervical ripening in late pregnancy. Methods A total of 80 single term pregnant women with normal location but without any vaginal delivery taboo were randomly divided into 2 groups, 40 cases in study group receiving vaginal administration of 10mg dinoprostone, another 40 cases in control group receiving intravenous injection of 0. 5% oxytocin. Results Twelve hours after medication, the degree of cervical ripening in study group was significantly superior to that in control group （χ^2 = 20. 192 ,P 〈0.01 ）, and the average time to labor starting was obviously shorter （t = 3. 794 ,P 〈 0.01 ）. The rate of labor in study group within 24 hours was 57.5% , which was higher than 10.0% in control group （χ^2 = 5. 764 ,P 〈 0.05 ） . The success rate of induced labor in study group was 82.5% , which was significantly higher than 20.0% in control group （χ^2 = 19. 431 ,P 〈 0.01 ）. There was no significant difference in total labor stages and neonatal outcomes （ both P 〉 0.05 ）. Conclusion Dinoprostone can be used in promoting cervical ripening and induced labor safely and effectively.
