摘要 目的:开发一款符合国际标准和临床使用习惯、适合回顾性及前瞻性临床研究、科学合理地对脑卒中临床数据进行管理,并能进行数据挖掘和其他分析的软件系统,满足脑卒中防控筛查需要。方法:根据卫生部脑卒中筛查与防治工程委员会的脑卒中CRF表模板作为主要核心数据内容,结合我院临床工作需要,确定脑卒中研究的数据项目。重新设计科学的脑卒中数据管理架构,采用Visual Basic 6.0、Vista DB等工具和组件进行开发编程。结果:经过6个多月的开发和完善,联合我院神经外科、神经内科和信息科三个科室进行数据结构讨论,录入近百例脑卒中临床数据资料,建立起包含近600项临床指标、内置数据库挖掘和分析功能的脑卒中临床数据信息化系统,各项指标兼容卫生部信息化要求,功能和实践检验达到设计目标。该软件已与全国5家医院达成合作并成功运行。数据挖掘功能达到预期目标,能可视化地进行复杂搜索、统计分析和对数据描述性分析。记录和数据的导入、导出满足了协作组之间数据交流的需要,导出任意搜索和统计数据为文本文件,方便进一步提交到大型统计分析软件。结论:本系统符合卫生部相关脑卒中数据方面的规定,同时具备较好的创新,在脑卒中筛查防控等领域能发挥重大作用,具有广泛的应用前景。 Objective: to meet the demand of stroke prevention and screening, develop a set of software that meets international standards and established clinical conventions, is adequate to retrospective and prospective clinical study, is able to scientifically management the clinical data of stroke, and can carry out data mining and other analysis. Methods: set the stroke CIKF template of Stroke Screening and Prevention Engineering Council of Ministry of Health as core content, and combined with the actual clinical demand in our hospital, it confirmed the data item of stroke study. It redesigned a more scientific framework for stroke data management, and tools and components, including Visual Basic 6.0, Vista DB, etc, were adopted in programming. Results: after six months development and improvement, and combined with the data structure discussion of Dept of Neurosurgery, Dept of Neurology and Dept of Information, it recorded the chnical data of nearly 100 stroke cases and established a clinical stroke data information system with nearly 600 clinical indexes, built-in data mining and analysis functions. The software has been successfully applied in 5 hospitals in China. It can not only carry out data mining as expected, but also complete complicated search, statistical analysis and descriptive analysis for data visually. The recording and data importing, exporting met the demand of data communication between cooperative groups, and the exported search and statistical data were text, which is convenient for submitting to larger statistical analysis sotS:ware Conclusions: the software can meet relevant regulations on strokes made by the Ministry of Health, and meanwhile, it is innovative will play an important role in stroke screening and prevention field, and has a broad perspective in application.
