摘要
目的探讨胃食管反流病与幽门螺杆菌感染之间的相关性。方法将经过电子胃镜确诊的GERD患者120例及对照组轻度慢性浅表性胃炎患者120例予血清幽门螺杆菌抗体检测和14C呼气试验法进行H.pylori检测,对比两组H.pylori感染情况;将90例反流性食管炎患者分为LA-A、B组及LA-C、D组,对比两组H.pylori感染情况;将120例GERD患者分为轻度症状组、中度症状组、重度症状组及极重度症状组,比较组间H.pylori感染情况。结果 GERD组H.pylori感染的阳性率(39.17%)低于对照组H.pylori感染的阳性率(62.50%),差异有统计学意义(P<0.05)。LA-A、B组H.pylori感染的阳性率(60.87%)高于LA-C、D组H.pylori感染的阳性率(29.55%),差异有统计学意义(P<0.05)。轻度症状组、中度症状组、重度症状组及极重度症状组H.pylori感染的阳性率分别是40.00%、41.67%、40.63%、31.82%。结论幽门螺杆菌感染是反流性食管炎的保护因素,幽门螺杆菌感染与GERD症状的发生无相关性。
Objective To investigate the relationship between gastroesophageal reflux disease (GERD) and infec- tion of helicobacter pylori (H. pylori). Methods 120 patients with GERD and 120 patients with low-grade chronic su- perficial gastritis diagnosed through endoscopy were checked by testing H. pylori antibody of serum and 14C-UBT. The H. pylori infection between experimental group and control group was compared. 90 patients with reflux esophagitis (RE) were derided into LA-A, B group and LA-C, D group. The infection rate of H. pylori was compared. 120 patients with GERD were divided into mild group, moderate group, severe group and the most severe group by the GERD symp- tom score. The infection rate of H. pylori was compared among the groups. Results The infection rate of H. pylori in GERD (39.17%) was remarkably lower than that in control group (62.50%), the difference was marked (P 〈 0.05 ). The infection rate of H. pylori in LA-A, B group (60.87 % ) was higher than that in LA-C, D group (29.55 % ). The difference was marked (P 〈 0.05). The infection rates of H. pylori in mild group, moderate group, severe group and the most severe group were 40.00% , 41.67% , 40.63% and 31.82%. Conclusion The H. pylori infection is a protective factor in the development of reflux esophagitis ( RE). The H. pylori infection does not seem to play a major role in producing typial GERD symptoms.
