KUS股骨近端锁定钢板治疗老年股骨粗隆间骨折

Treatment of Femoral Intertrochanteric Fractures with KUS Locking Compression Plates in Old Patients

目的:L探讨应用KUS股骨近端锁定钢板治疗老年股骨粗隆间骨折的治疗效果。方法:回顾性分析我院自2009年05月至2010年04月收治的36例老年股骨粗隆间骨折患者的临床资料,所有患者均行切开复位、KUS股骨近端锁定钢板内固定治疗;术后6周、3月、6月、12月时常规随访,以后每6月随访;记录术后骨折愈合时间及髋关节功能情况。结果:所有患者均获随访,时间5～39个月,平均18个月;骨折均愈合,时间13～20周,平均15周。术后髋关节按Harris标准评分:优24例,良10例,中2例,差0例,总优良率94.4%,均未出现骨折再移位、内固定断裂及失败、髋内翻畸形、股骨头坏死等并发症。结论:KUS股骨近端锁定钢板治疗老年股骨转子间骨折符合生物力学固定原则,是治疗老年粗隆间骨折的良好选择。

Objective：To evaluate the clinical efficacy of KUS locking compression plate（LCP） on femoral intertrochanteric fractures in old patients.Methods：Thirty six old patients with femoral intertrochanteric fractures were treated with KUS LCP internal fixation.Their ages ranged from 65 to 94 years,averaging 74 years.They were 15 males and 21 females.By AO classification,11 cases were type A1,20 cases were type A2,5 cases were type A3.All patients were followed up at the sixth week,3rd,6th,and 12th month to record fracture healing time and recovery of function after operation.Results：All the patientswere followed up for 5 to39（mean,18） months.Fracture healing time was 13 weeks to 20 weeks,averagely 15 weeks.No fracture redisplacement,failure of internal fixation,coxa vara deformity,and avascular necrosis of femoral head was observed.On the base of Harris score system,the functions of hip joint were excellent in 24 cases and good in 10 cases.Conclusion：KUS LCP coincidence biological osteosynthesis（BO） principle,has a preferable clinical value in the treatment of aged patients with femoral intertrochanteric fracture.
