摘要
背景:伤科消炎膏能够缓解膝关节骨性关节炎患者膝关节滑膜炎症,显著改善膝关节肿胀和疼痛,但对于其作用机制目前尚缺乏研究。作者通过动物实验观察其对膝关节滑膜中白介素-1(IL-1)、白三烯(LT)、金属蛋白酶-3(MMP-3)浓度的影响以期初步探讨其作用机制。目的:观察伤科消炎膏对木瓜蛋白酶诱导兔膝骨关节炎性模型的膝关节滑膜中IL-1、LT、MMP-3浓度的影响。方法:通过木瓜蛋白酶关节腔内注射建立18个兔右膝骨关节炎性模型,然后将其随机分为空白组、伤科消炎膏组和双氯芬酸二乙胺组,每组6只。空白组不进行治疗,伤科消炎膏组外用伤科消炎膏治疗、双氯芬酸二乙胺组外用双氯芬酸二乙胺治疗。于治疗后第11天用ELISA法检测试验动物右膝关节滑膜组织中IL-1、LT、MMP-3的浓度。结果与结论:空白组滑膜IL-1浓度高于伤科消炎膏组(P<0.05),空白组滑膜LT浓度高于双氯芬酸二乙胺组(P<0.05),各组间滑膜MMP-3浓度差异无显著性。本研究结果提示伤科消炎膏系通过降低滑膜组织中部分与炎性反应相关的细胞因子水平来缓解膝关节骨性关节炎的滑膜组织炎性反应,其作用机制与双氯芬酸二乙胺类似,但作用途径与双氯芬酸二乙胺相比有所不同。伤科消炎膏对炎性滑膜组织细胞因子中的IL-1具有影响,而双氯芬酸二乙胺对炎性滑膜组织细胞因子中的LT具有影响。但实验结果未显示伤科消炎膏对于滑膜MMP-3浓度有降低作用,说明该药物在抑制滑膜组织胶原降解方面作用不显著。
Background:Shangke Xiaoyan Plaster can obviously relieve synovitis,alleviate the swell and pain of patients with knee ostearthritis.But its action mechanism is still unclear.We observe the levels of interleukin-1(IL-1),leukotriene(LT)and Matrix Metalloproteinases-3(MMP-3)in Papain-induced rabbit knee osteoarthritis after the therapy of Shangke Xiaoyan Plaster and then illustrate the action mechanism.Objective:To observe the influence of Shangke Xiaoyan Plaster on synovium IL-1,LT and MMP-3 levels in Papain-induced rabbit knee osteoarthritis.Methods:18 Papain-induced rabbit knee osteoarthritis models were divided into blank control group,Shangke Xiaoyan Plaster group and diclofenac diethylamine group randomly.The latter two groups were treated with Shangke Xiaoyan Plaster and diclofenac diethylamine separately.The ELISA method was used to investigate the IL-1,LT and MMP-3 levels of synovium in Papain-induced rabbit knee osteoarthritis 11 days after the therapy.Results and Conclusion:The IL-1 level of blank control group was higher than that of Shangke Xiaoyan Plaster group(P0.05).The LT level of blank control group was higher than that of diclofenac diethylamine group(P0.05).There was no difference with the MMP-3 level in each group(P0.05).The results indicated that Shangke Xiaoyan Plaster can alleviate the synovitis of knee osteoarthritis by decreasing the content of some inflammation-related cytokine in synovial membrane.Its mechanism was similar to diclofenac diethylamine.But the difference was that Shangke Xiaoyan Plaster had effect on the IL-1 of inflammatory synovial membrane while diclofenac diethylamine had effect on the LT of inflammatory synovial membrane.But no evidence showed that Shangke Xiaoyan Plaster can decrease the MMP-3 level of synovium.This indicates there is no significant effect of Shangke Xiaoyan Plaster on inhibiting collagen degradation of synovium.
