女性2型糖尿病与睡眠相关行为的相关性研究 被引量:5

摘要 目的研究女性2型糖尿病(T2DM)与睡眠相关行为(午睡情况、晚睡情况、打鼾情况、入睡情况、夜间睡眠充足与否及睡眠质量)之间的关系。方法采取以医院为基础的病例对照研究。病例为首次确诊的女性T2DM患者,对照者为同期住院并住同一医院的非T2DM及相关代谢性疾病女性患者。病例与对照各192例,按1∶1匹配。采用国际匹兹堡睡眠质量指数量表(PSQI)和自行设计的一般状况调查表对两组研究对象进行调查问卷。采用t检验及条件Logistic回归模型对睡眠质量及睡眠相关行为与女性T2DM之间的关系进行分析。结果病例组与对照组睡眠质量(PSQI总分)比较,差异有统计学意义(t=7.60,P<0.01)。多元条件Logistic回归显示:睡眠相关行为——打鼾情况、睡眠质量、夜间睡眠情况及午睡情况与T2DM发病存在关联〔OR值分别为5.749、5.738、3.923、3.600;95%CI分别为(3.034,10.275)、(3.421,9.624)、(2.413,6.378)、(2.197,5.899)〕。结论女性睡眠相关行为与T2DM发病密切相关,午睡缺乏、打鼾、夜间睡眠不足、睡眠质量差可能是女性T2DM发病的危险因素,适当午睡可能是女性T2DM发病的保护性因素。 Objective To investigate the correlation between female type 2 diabetes meUitus (T2DM) and sleep -re- lated behaviors (nap, stay up, snoring, difficulty in sleeping, not enough sleep at night and sleep quality). Methods A hospital- based case- control study was conducted. The cases were females first diagnosed with T2DM, and the controls were females admitted to the same hospital in the same period without T2DM and metabolism diseases. The cases and controls were both 192 and matched by 1 : 1. International Pittsburgh Sleep Quality Index (PSQI) and self- designed general condition investi- gation questionnaire were used to survey all subjects. The correlation between sleep quality, sleep - related behaviors ( nap, stay up, snoring, difficulty in sleeping, not enough sleep at night and sleep quality) and female T2DM was analyzed by t -test and conditional Logistic regression models. Results The sleep quality (total PSQI scores) between the case group and control group showed statistically significant difference ( t = 7.60, P 〈 0. 01 ). Multivariate conditional logistic regression analysis showed that sleep -related behaviors (snoring, sleep quality, not enough sleep at night and nap) were correlated with the oc- currence of T2DM [ OR was 5.749, 5. 738, 3. 923 and 3. 600 respectively, 95% CI was ( 3.034, 10. 275 ), ( 3.421, 9. 624), (2. 413, 6. 378), (2. 197, 5. 899) respectively]. Conclusion Sleep - related behaviors are closely related to the occurrence of T2DM in females. Lack of nap, snoring, not enough sleep at night and bad sleep quality might be risk factors for female T2DM and adequate nap might be protective factors for female T2DM.
