贝前列素钠对老年糖尿病肾病患者肾功能及凝血功能的影响

Impact of Beraprost Sodium on Renal Function and Blood Coagulation of Elderly Patients with Diabetic Nephropathy

目的观察贝前列素对老年糖尿病肾病患者的肾功能及凝血功能的影响。方法选择40例老年糖尿病肾病患者,采用随机数字表分为治疗组和对照组,每组20例。两组均给予控制血糖、血压、血脂等常规治疗及对症处理,治疗组在基础治疗上给予贝前列素钠联合复方α-酮酸片治疗;对照组在基础治疗上给予复方а-酮酸片治疗。两组均30 d为1个疗程,连续治疗2个疗程后,观察患者的肾功能指标(血尿素氮、血肌酐)、24 d尿蛋白、凝血指标(纤维蛋白原、D-二聚体)的变化及不良反应并比较。结果治疗前两组患者的血尿素氮、血肌酐、24 h尿蛋白定量、血浆D-二聚体及FIB水平比较,差异均无统计学意义(P>0.05);而治疗后两组患者上述各指标比较,差异均有统计学意义(P<0.05)。结论老年糖尿病肾病患者加用贝前列素钠治疗可降低血浆纤维蛋白原、血浆D-二聚体、血肌酐、血尿素氮及24 h尿蛋白水平,改善患者的内皮功能和纤溶活性,加快肾脏清除毒素的能力,延缓糖尿病肾病进程。

Objective To investigate the impact of beraprost sodium on renal function and blood coagulation of elderly patients with diabetic nephropathy （DN）. Methods Forty elderly DP patients were divided randomly into G1 （ therapeutic group） and G2 （control group）, 20 in each. Both groups were given a basic treatment, and based on it, G2 treated with compounder -keto acid tablets, G1 combined with additional beraprost sodium. The therapeutic results and side effects were e- valuated. Results Clinical symptoms were improved significantly and levels of blood urea nitrogen （ BUN）, serum creatinine （ Scr）, blood FIB, urinary protein decreased significantly in G1 after treatment （ P 〈 0. 05 ）. Conclusion In aged DP pa- tients, beraprost sodium can lower the levels of BUN, Scr, D - dimer, FIB, 24 h urine protein, and improve endothelial func- tion and fibrinolytic activity, accelerate the ability of kidneys removing toxins, slow down the process of DP.
