肥胖与代谢综合征患者血清lipocalin-2、hsCRP变化 被引量:7

A Study on the Changes of Serum Lipocalin-2 and High Sensitive C Reactive Protein Levels and Their Relationship in Obese Patients and Metabolic Syndrome Patients

摘要 目的:探讨肥胖、代谢综合征患者血清中性粒细胞明胶酶相关脂质运载蛋白2(lipocalin-2,LCN2)、超敏C反应蛋白(hsCRP)的变化及其相关性。方法:选择104例正常体重者、107例超重者和52例肥胖者进行体格检查、口服葡萄糖耐量试验及血脂、胰岛素(FINS)、hsCRP、lipocalin-2检测;计算胰岛素抵抗指数(HOMA-IR)。结果:(1)肥胖患者血清lipocalin-2增高,校正年龄和性别后,血清lipocalin-2水平与体重指数(BMI)、腰围、体脂含量、hsCRP正相关(P<0.05),与HDL-C水平负相关(P=0.001);(2)校正年龄、性别和BMI后,血清lipocalin-2水平仍与hsCRP正相关(P<0.05),与HDL-C水平负相关(P=0.023);多元逐步回归分析显示,hsCRP是血清lipocalin-2的独立决定因素;(3)代谢综合征患者lipocalin-2水平增高,但lipocalin-2与代谢紊乱数目未见显著性关联。结论:肥胖与代谢综合征患者血清lipocalin-2和hsCRP均增高,且lipocalin-2水平与hsCRP正相关。lipocalin-2可以作为肥胖相关低度炎症的新的标志物。 Objective: To explore the changes of serum lipocalin-2(LCN2) and high sensitive C reactive protein(hsCRP) levels and their correlation in obese patients and metabolic syndrome(MS) patients.Methods: Total 104 normal control subjects,107 overweight subjects and 52 obese patients were selected.Physical examination and oral glucose tolerance test(OGTT) were performed in the three groups,and serum lipid,fasting insulin,hsCRP,serum lipocalin-2 levels were measured.Insulin resistance index(HOMA-IR) was calculated.Results:(1)The lipocalin-2 levels of obese patients were higher than those of normal controls and overweight subjects.After adjusting age and sex,lipocalin-2 level was positively correlated with BMI,waist circumference(WC),body fat content and hsCRP level(P0.05),but was negatively correlated with HDL-C level(P=0.001).(2)After adjusting sex,age and BMI,lipocalin-2 level was also positively correlated with hsCRP level(P0.05) and was negatively correlated with HDL-C level(P=0.023).Multiple stepwise regression analysis demonstrated that hsCRP was independent determinant for lipocalin-2 level.(3)Lipocalin-2 level increased in MS patients,but was not significantly associated with the numbers of metabolic disorders.Conclusions: Serum lipocalin-2 and hsCRP levels significantly increase in obesity patients and MS patients.Lipocalin-2 level positively correlates with hsCRP level.Lipocalin-2 might be a useful new marker for evaluating obesity-related low-grade inflammation.
