西洛他唑联合前列腺素E1治疗2型糖尿病合并下肢动脉病变早期疗效观察

Clinical effects of cilostazol combined with prostaglandin E1 in the treatment of type 2 diabetes complicated with peripheral artery disease of lower extremity.

目的探讨西洛他唑联合前列腺素E。治疗2型糖尿病合并下肢动脉病变（PAD）的早期疗效。方法2型糖尿病合并PAD早期患者60例，按就诊时间先后次序随机分为研究组和对照组各30例。在常规给予饮食干预、运动治疗以及降糖药物治疗基础上，对照组前列腺素E1注射液10鹇加入生理盐水100ml中静脉滴注，研究组在对照组基础上口服西洛他唑片100mg，bid。两组疗程均为1个月。检测并比较治疗前、后两组的踝肱指数（ABI）、脉搏波传导速度（PWV）、临床症状评分，比较两组治疗有效率。结果两组治疗后ABI、PWV评分均较治疗前明显升高，差异有统计学意义（P〈0．05-0．01），临床症状评分较治疗前明显降低，差异有统计学意义（P〈O．01）；治疗后研究组PWV评分及治疗有效率较对照组明显升高，临床症状评分较对照组明显降低，差异均有统计学意义（P均〈0．05）。结论西洛他唑联合前列腺素E，治疗2型糖尿病合并PAD患者，具有协同作用，可明显改善下肢动脉病变，不增加出血倾向，疗效可靠，值得临床推广应用。

Objective To investigate the clinical effects of cilostazol combined with prostaglandin E1 in the treatment of type 2 diabetes complicated with peripheral artery disease （PAD）. Methods A total of 60 patients of type 2 diabetes complicated with PAD of lower extremity were randomly assigned into the study group and the control group, with 30 cases in each group. Based on routine dietary intervention, exercise therapy and hypoglycemic drugs, the control group was also given intravenous infusion of 10/~g prostaglandin Et added into normal 100 mL saline, while the study group was given an extra oral cilostazol tablets, 100 mg, bid. Treatment course for both groups were one month. Ankle-brachial index （ABI）, pulse wave velocity （PWV）, ratings of clinical symptoms and effective rate were detected and compared between the two groups. Results After treatment, both groups showed significant in- crease in ABI and PWV （P〈0.05~0,01） and significant decrease in ratings of clinical symptoms, compared with those before treatment （P〈0.01）. After treatment, the study group showed significantly higher PWV and effective rate but significantly lower rating of clinical symptom than the control group （P〈0.05）. Conclusion Cilostazol combined with prostaglandin E, can ameliorate lower extremity arterial disease without increasing bleeding tendency, thus is effective for treating patients with type 2 diabetes complicated withPAD.
